Work Schedule: Days and Evenings with Weekend and Holiday Rotation

Summary:

The social worker, as an active member of the interdisciplinary Emergency Department (ED) team, provides comprehensive clinical social work, care coordination and discharge planning services to at-risk patients and their families in the Emergency Department. The social worker utilizes strong biopsychosocial assessment skills, advanced problem solving and care planning, and a variety of counseling modalities to coordinate optimal follow up and transitional care. The social worker works collaboratively with other members of the ED team, care management team, community resources, regulatory agencies and payers to facilitate care transitions while maintaining the highest quality of services possible.

Responsibilities:

EDUCATION

* Master of Social Work (MSW) from a school accredited by the Council on Social Work Education
* Current NYS Master of Social Work license (LMSW) or current NYS Clinical Social Work license (LCSW) preferred
* NYS Screen Certification preferred

EXPERIENCE

* Two years social work experience in a health related setting
* One of the two years may be an academic year as a graduate social work intern in a health related setting
* Experience working on an interdisciplinary team
* Experience working with persons considered to be high risk due to health conditions including behavioral and substance use and/or psychosocial concerns

KNOWLEDGE, SKILL AND ABILITY

* Excellent skills in conducting a rapid biopsychosocial assessment, evaluation of readiness to change, and crisis intervention
* Experience with various assessment tools for depression, substance use, and social determinants of health
* Experience with multiple counseling modalities including motivational interviewing, and solution focused intervention
* Knowledge of relevant resources and services commensurate with patient illness, impairments in physical functioning, and social determinants of health needs

WORKING CONDITIONS

* Hours may vary at times to meet the needs of patients/families, the ED and Care Management Departments
* May be asked to cover other sites or other units on occasion

ENVIRONMENT

* Normal heat, light space, and safe working environment typical of most office jobs; and
* Occasional exposure to one or more mildly unpleasant physical conditions
