Job Summary:

Baptist Health Pharmacy Services are committed to ensuring the optimal, safe, and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value based environment.

Responsible for assisting pharmacists with obtaining and recording prior to admission medication histories and patient allergy lists in the hospital's health information system.

Determines patient compliance to medication regimens and presence of high-risk medications or allergy interactions, and communicates concerns to pharmacist and/or healthcare providers as appropriate. Collects data to track service specific metrics and productivity. Trains pharmacists, pharmacy residents, pharmacy technicians, and pharmacy students as required.

May periodically be responsible for performing basic pharmacy technician duties including but not limited to delivering medications from central pharmacy, restocking and expiring medications from automated dispensing system (ADS) cabinets, triaging ADS cabinet problems.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job. He or she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

1. Obtains an accurate medication history using reliable sources, including interviewing the patient and/or family; contacting the patient's community pharmacy, mail order pharmacy, long term care facility, and/or provider's office; reviewing previous medical records; and/or utilizing Kentucky All Schedule Prescription Electronic Reporting (KASPER) reports for controlled substances. Appropriately documents patient's compliance to medication regimen in the hospital's health information system.

2. Creates a complete and accurate medication list in the hospital's health information system, including medication name, dose, strength, frequency, route and time of last administration for all prescription, over-the-counter, and herbal medications as well as for professional medication samples.

3. Triages workflow based on patient acuity, likelihood of admission, and medication regimen complexity if known.

4. Obtains and documents patient's allergies and reaction details in the hospital's health information system.

5. Communicates issues such as noncompliance, high-risk medications, duplicate therapy, and/or new allergies to the pharmacist or other healthcare professionals, as appropriate.

6. Can perform the basic functions of an inpatient pharmacy technician, including but not limited to:

* Accurately restocking and expiring medications from the ADS
* Triaging basic ADS cabinet issues
* Delivering medications in a timely manner when needed
* Fills prescriptions within State and Federal laws for checking by a pharmacist
* Can perform basic functions on the pharmacy automation systems including the Med Robot and the Med Carousel.
* Triaging Pharmacy Department phone calls

7. Trains pharmacists, pharmacy residents, pharmacy technician, and pharmacy students on job duties as required.

8. Uses technology proficiently, including the hospital's health information system, Connect-Rx, ADS cabinets, and Microsoft Outlook, Microsoft Word, and Microsoft Excel.

9. Collects data on specified metrics to present to either the assigned Clinical Pharmacist or Pharmacy Manager

10. Other projects or tasks as assigned by the Director of Pharmacy, Pharmacy Manager, or Pharmacy Technician Supervisor.

Minimum Education, Training and Experience Required:

A. High School diploma or equivalent preferred.

B. Pharmacy Technician experience preferred.

C. Required Kentucky Board of Pharmacy registration certificate for Pharmacy Technician or Pharmacy Intern. (Within 30 days of hire date if you are a new registrant. If previously registered you must have registration upon hire).

D. Required certification with the Pharmacy Technician Certification Board (PTCB) or the Institute for the Certification of Pharmacy Technicians (ICPT). (Within 180 days of hire date) unless licensed as a Pharmacy Intern.
