The social worker provides comprehensive social work and discharge services to patients and families in the acute care setting and emergency room. As an active member of the interdisciplinary team the social worker affects a process of assessment, planning, facilitation and advocacy to meet a patient's bio-psychosocial needs with attention quality, thoughtful use of resources and optimum patient experience. The social worker, while demonstrating the values and mission of Catholic Health, builds and maintains professional, resource based relationships across the continuum of care and within the community.



* Bachelors of Social Work (B.S.W.) from a school accredited by the Council on Social Work Education or
* Masters of Social Work (M.S.W) from a school accredited by the Council on Social Work Education with a B.A. or B.S. in another field
* New York State Screen certification preferred


* Minimum one (1) year B.S.W. social work experience in an acute care hospital preferred
* One (1) year experience as a B.S.W. in a sub acute, long term care or health related setting or
* One (1) academic year experience as a social work intern in an acute hospital, medical rehab unit, or sub-acute unit in a nursing home


* Knowledge of the discharge planning process, levels of post acute care and a wide range of community resources
* General knowledge of community resources to assist maternal child patient populations
* Able to work within the role of a social worker on an interdisciplinary team
* Able to manage a high volume of psychosocially complicated cases and concurrently maintaining optimal patient/family experience
* Excellent interpersonal, communication, networking, and advocacy skills


* Varied hours and days at times to meet the needs of patients and families
* Occasional travel between Catholic Health facilities as needed


* Normal heat, light space, and safe working environment typical of most office jobs; and
* Occasional exposure to one or more mildly unpleasant physical conditions.
