Scheduled Hours per 2 week Pay Period: 80

Primary Location: AR > LITTLE ROCK > LRIMC

Minimum Required Skills, Knowledge, and Experience:

Current licensure in good standing as a Licensed Clinical Social Worker in Arkansas with a minimum of two years of clinical experience

Ability and willingness to provide clinical supervision to graduate and/or undergraduate social work students in clinical rotations designed to train them in integrated behavioral healthcare (applicant must have already completed, or be willing to complete, the requisite training to qualify as a field instructor for the UALR School of Social Work)

Established competency in screening and brief assessment of mental/behavioral health disorders, and ability to triage patients' needs to identify the safest and most appropriate clinical responses

Established competency in providing brief cognitive and behavioral interventions to individuals, families, couples, and groups with identified behavioral health/substance abuse concerns

Excellent interpersonal skills and role adaptability in order to collaborate with a multidisciplinary team of physicians, nurses, population health coaches, other behavioral health clinicians and administrative staff in a primary care medical setting

Primary Function(s):

The Integrated Behavioral Health Provider (IBHP) will be an essential part of the primary care team in their identified clinic(s), acquiring referrals almost solely from the population of patients whose primary care treatment needs are already being served by the clinic's CHI St. Vincent PCPs. The IBHP will communicate their professional opinions to PCPs concerning the mental health needs of patients, how these needs might best be addressed, and how mental, behavioral, and/or psychosocial variables may be adversely impacting patients' medical conditions or compliance with prescribed treatment. IBHPs will provide brief therapeutic interventions (1-5 visits) to address patients' identified mental health concerns, and refer to specialty mental health if longer-term intervention is clinically indicated. IBH providers may co-manage the emotional and behavioral components associated with patients' identified health issues (e.g., diabetes, heart disease, obesity, etc) with the primary care team to maximize positive health outcomes. The IBHP's office space will be located in a CHI St. Vincent primary care medical clinic. The IBHP will be considered as an employee of the St. Vincent Medical Group, under the clinical direction of the CHI St. Vincent Outpatient Behavioral Health department.

Primary Tasks and Responsibilities

Brief Psychotherapeutic Consultations, typically in 15-45 min sessions- Productivity expectations of an IBHP are to build to an average of 8 face-to-face patient interactions per clinical day, which may include any of the following:

Brief psychotherapeutic interventions with individuals, couples, or families focused on improving patients' problem-solving skills, modifying negative thinking, and promoting self-efficacy

Brief assessment or screening of mental/behavioral health symptoms

Timely assessment and treatment recommendations for risk of self-harm or aggression

Psychoeducation for patients and/or their support systems to improve coping, frustration tolerance, stress reduction, and relaxation skills

Medication adherence and disease management counseling

Motivational Interviewing to develop behavioral strategies aimed at symptom reduction and improved physical health

Self-Care Plan development and skills training to facilitate self-disease management

Brief substance use/abuse evaluation and clinically appropriate follow-up

Consultation with PCPs to enhance their understanding and ability to address the needs of the whole patient, provide decision support for primary care treatment planning, and provide brief interventions to support PCPs' treatment objectives.

Coordination of Patient Care- Provide consultation to and coordinate care of patients with primary care staff. Identify, triage, and advocate for patients needing more intensive or specialty mental health services.

Crisis Intervention - Be immediately available to the primary care team during working hours to conduct brief examinations for patients verbalizing or displaying risk for self-harm or aggression toward others.

Documentation - Complete all charting as needed and required by CHI St. Vincent billing practices,

including results of evaluations/assessments and clinical progress notes, utilizing the clinic(s) electronic health record system (software-specific training to be provided after hire if applicant has no prior experience)

Other Duties:

Assist in providing training and education in integrative BH primary care model.

Participate in behavioral health quality improvement standards.

Participate in behavioral health trainings and conferences.

Note: Each IBHP will receive structured training in CHI St. Vincent's Integrated Behavioral Healthcare model and its implementation at the beginning of their employment
