Job Description

Summary:

The Clinical Pharmacist reads and interprets all medication orders and prescriptions received by the pharmacy. Additionally, the clinical pharmacist compounds and dispenses medications and supervises pharmacy technicians and interns who do the same. The clinical pharmacist will also assist with various clinical activities including rounding, attendance at CODE/Trauma cases, reviewing Clinical/Operational reports for appropriate, cost effective therapeutic interventions, and staffing pharmacy outpatient services including anticoagulation, diabetes and MTM clinics. The clinical pharmacist will also assist with precepting pharmacy students, educating nursing and/or medical staffs and assist with quality efforts through serving on and preparing materials for hospital committees/initiatives as requested.

Responsibilities:

Competent in the Practice of Pharmacy: Does not have high error rate when entering orders, making IV medications or checking Automated dispensing cabinet refills. Supervises the effective delivery of pharmacy services. Performs charge and on-call duties effectively Responds appropriately to emergency and problem situations, including Codes. Encourages cost effective practices and implementation of patient care

Controlled Substances Inventory and Record Keeping Dispenses narcotics/controlled substances according to hospital policy. Performs monthly narcotic count if asked by Director, and verifies count at each dispense from safe Complies with all required filing of reports. Corrects any discrepancies in controlled substance manager or variances on automation bs. controlled medication exception report immediately upon discovery. Forwards all incorrectly resolved discrepancies, controlled substance over-ride medications removed without an order, outstanding transaction reports, or other suspicious activity to the attention of the Manager.

Sterile Product Preparation Complies with required clean room attire and cleaning procedures. In the event of a technician or intern not complying, immediate corrective action is taken. Ensures that all required cleaning and monitoring logs are completed and documented on a daily basis. Successfully passes annual sterile product preparation competency and media fill test Assigns accurate beyond use dating labels to all IV compounds

Professional Conduct and Leadership Develops effective working relationships with pharmacy staff, nursing staff, medical staff and administration. Maintains positive attitude with respect to all hospital and departmental goals and objectives. Effectively directs technicians and interns, assuming charge dutieswhen required Reports to work on time, avoids excessive unscheduled absences. Maintains professional appearance and dress. Maintains confidentiality of all hospital and patient information. Maintains respect for patients and families.

Supports Hospital Information Systems and Maintains Educational and Safety Compliance Acts as resource for nursing, pharmacy and other staff for issues related to Pharmacy Automation or the pharmacy information system. Assists with troubleshooting of Automation problems, particularly on off shifts Completes all assigned competencies and continuing education requirements. Attends all required safety and educational programs Has working knowledge of all hospital and departmental policies. Can quickly find the answer to questions asked by accreditation surveyors. Is able to locate all required safety and clinical literature, including MSDS sheets.

Clinical Practice Documents and completes all interventions, maintaining an adequate number each month as determined by the Pharmacy Manager. Reviews daily clinical reports for cost saving and quality interventions, taking action as needed. Attends and participates in interdisciplinary rounds if assigned by Pharmacy Leadership. Fully supports pharmacy services, including the anticoagulation clinic, medication reconciliation, rounding, diabetes counseling service or other new clinical initiatives.

Educational Initiatives and Presentations Takes an active role in the precepting of pharmacy students from the various schools of Pharmacy. Enthusiastically orients and trains new employees. Prepares and presents one educational program to nursing, physicians or the community annually. Attends or contributes to pharmacy clinical or operational committees on a regular basis.

Operational Excellence Supports hospital and departmental efforts to achieve and maintain accreditation with all applicable regulatory bodies including The Joint Commission and RI DOH. Strives to constantly improve the quality of care delivered. Sets achievable goals for self improvement and professional growth. Assists with quality improvement projects, such as Drug Use Evaluations, analysis of interventions etc as requested by Pharmacy Leadership.

Other information:

EDUCATION: Doctor of Pharmacy Degree or Bachelors of Science in pharmacy from an accredited college of pharmacy.

WORK EXPERIENCE: Two years experience as a hospital pharmacist preferred. Certification in one or more specialty areas such as Anticoagulation, Diabetes Management, Ambulatory care, MTM, pain managemet, Pharmacotherapeutics, Healthcare IT systems or Nutrition Support strongly encouraged.

LICENSES/CERTIFICATIONS: Current licensure as a pharmacist by the Rhode Island Board of Pharmacy

BLS, ACLS, and PALS certification strongly encouraged.

Membership in a professional organization preferred.

REQUIRED SKILLS: Ability to interact professionally with patients, visitors, physicians, and members of all hospital departments.

In addition, ability to prepare and present educational programs for patients, physicians and other allied health departments.
