Job Description

The Medicaid Managed Care Organization (MMCO) Foster Care Liaison will be responsible for ensuring high touch coordination between Office of Children and Family Services (OCFS), Local Department of Social Services (LDSS), and (if applicable) the Voluntary Foster Care Agency (VFCA) for all children enrolled in foster care. This liaison must also have experience, expertise, and knowledge of the child welfare system, foster care healthcare requirements and the unique complex needs of this population.

Reports to: Mgr. Behavioral Health

Department: PHS-NY Care Management

Location: Manhattan, NY.

Level: L16

Essential Functions:

* Serve as the direct plan contact for agency coordinators and service providers, and be responsible for monitoring access for children in foster care.
* Resolves problems and provides guidance to local department of Social Services (LDSS) and Voluntary foster care agencies (VFCA).
* Responsible for driving trauma informed principles into the care coordination function within team and external agencies, translating these into meaningful processes and offerings for children in foster care.
* Facilitate resolution of LDSS and VFCA issues and concerns by phone, email or in person.
* Be readily available via telephone and email to the LDSS/VFCA during regular business hours to address any issues for managed care enrollees in Foster Care.
* Responsible for informing, consulting on the production of, and using data, reporting, and analytics for successful care coordination of children in foster care.
* Assist with enrollment, disenrollment, and access to care issues (including facilitation of single case agreements when a child is placed outside of the Plan's service area).
* Responsible for ensuring immediate issuance of a Welcome Letter or other temporary identification showing the effective date of enrollment of or replacement insurance identification card.
* Collaborate with case management and other internal departments.
* Work with Network in establishing provider contracts.
* Performs other duties as assigned.

Candidate Education:

* Required A Master's Degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field that provides a foundation to receive a license as required of the position
* Required Other or Registered Nurse with children's behavioral health background
* Preferred A Doctor of Philosophy (PhD) in a related field Psychology

Candidate Experience:

* Required 5 years of experience in of post-licensure experience in Psychiatric, chemical dependency or outpatient therapy with children.
* Preferred Other Experience working with children with unique complex needs (including needs resultant of trauma)

Candidate Skills:

* Intermediate Demonstrated written communication skills
* Intermediate Demonstrated interpersonal/verbal communication skills
* Intermediate Ability to work as part of a team
* Intermediate Ability to work independently
* Advanced Demonstrated organizational skills
* Intermediate Knowledge of medical terminology and/or experience with CPT and ICD-10 coding
* Intermediate Other Knowledge of foster care healthcare requirements

Licenses and Certifications:

A license in one of the following is required:

* Required Other One of the following licenses is required: LCSW, LMSW, LMHC, LFMT or RN. License must be unrestricted and active in the state of New York.
* Preferred Other Psy D License
