Job Summary and Scope

The Inpatient Staff Pharmacist ensures the safe and effective use of medication by providing medication order verification while assessing for optimal drug product selection, dosage regiment design, effective administration and drug therapy monitoring services at SRMC. The pharmacist monitors inpatient and outpatient drug therapies and works with the appropriate medical staff to find alternatives when needed. This staff member works closely within a multidisciplinary team to ensure the best care is provided. The pharmacist supervises the pharmacy technicians and interns in the performance of their duties. This position is responsible for providing the best customer service to both internal and external customers. The pharmacist ensures adherence to State and Federal regulations as well as Hospital and Departmental policies and procedures. The pharmacist will display a commitment to the hospital mission statement and role model the SRMC Values and consistently demonstrate in thought and action the SRMC commitment to putting you first.

Essential Functions:

* POLICIES AND PROCEDURES - Maintain established departmental policies and procedures, objectives, and quality assurance programs
* PROFESSIONAL DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops.
* Interprets, evaluates and accurately processes medication orders in a timely fashion. Performs order verification, screening for drug allergies, interactions, appropriateness of medication, dose, frequency, and route of administration. Additionally the order verification process entails screening for therapeutic duplication, variations from approved indications for use, and other contraindications.
* Actively identify, mitigate, prevent, and report potential and actual drug-related problems or adverse drug events. Recommend rational therapeutic alternatives as needed to prevent future problems.
* Performs pharmaceutical care duties including (but not limited to) anticoagulation dosing and monitoring, pharmacokinetic dosing, renal dosing of medications. Performs therapeutic interchanges per P&T Committee protocols. Acts as a collaborative healthcare team member by actively participating in medical teams and rounds.
* Provides supervision and oversight of the pharmacy technician and pharmacy intern staff in their daily activities.
* Provides patient education and medication counseling. Educates hospital and medical staff both individual and collectively.
* Responds to Code Blue, Rapid Response and emergency situations.
* Responds to on-demand or urgent requests for medications not stocked in the automated dispensing cabinets.
* Prepares sterile IV admixtures in accordance with 797 regulations. Prepares, in accordance with common standards of pharmacy practice, non-sterile compounds for patient use (examples: oral or topical dosage forms).
* Ensures medication security by using automated dispensing system, securing access to medications, adhering to all State and Federal regulations and Hospital and Departmental Policies and Procedures regarding Controlled Substances. Maintains an accurate paperwork accounting of all activities regarding Controlled Substances.
* Dispenses medications to the automated dispensing cabinetry and nursing units as needed. Performs basic automated dispensing cabinet maintenance as needed.
* Collects data and performs inspections for quality assurance purposes. Maintains all logs and necessary paperwork associated with regulatory and quality assurance requirements. Actively participates in LEAN processes and quality improvement initiatives.
* Perform miscellaneous job-related duties as assigned by the Director of Pharmacy

Required Qualifications

* Knowledge of pharmaceutical dosing forms, pharmaceutical and medical terminology, brand and generic names of medications.
* Knowledge of anatomy and physiology. Knowledge of pharmacology, clinical therapeutics, and pharmacokinetics are essential.
* Ability to integrate clinical services, distributive functions and education.
* Knowledge of sterile compounding techniques
* Knowledge in weighing, measuring and mixing medication ingredients
* Knowledge of inventory control and basic drug storage processes.
* Knowledge in State and Federal regulations,
* Ability to maintain records including, but not limited to, narcotic inventories and other controlled substance records.
* Knowledge of principles and processes for providing customer and personal services.
* Knowledge of arithmetic, algebra, statistics, and their applications. Knowledge of pharmaceutical calculations.
* Knowledge of electronic equipment, computer hardware and software, including applications
* Ability to be detailed oriented and consistently follow process
* Critical thinking skills, judgment and decision making abilities
* Ability to arrange things or actions in a certain order or pattern according to a specific rule or set of rules

Education/Experience

Education: BS in Pharmacy or PharmD

Licenses/Certifications: Current State of New Mexico Pharmacist License without limitations or restrictions

* BLS certification within 90 days of hire.
* ACLS certification within 90 days of hire.

Work Experience: Will consider a new graduate.

Preferred Educational/Experience Requirements:

* PharmD degree
* Residency in an acute care setting or
* 1 year hospital pharmacy experience.

Supervision

* Supervises certified pharmacy technicians, pharmacy interns and College of Pharmacy students in the course of their daily duties.

Conditions of Employment

* Must pass a pre-employment criminal background check, reference checks and a post offer drug screen.
* Must be employment eligible as verified by the U.S. Dept. of Health and Human Services Office of Inspector General (OIG) and the Government Services Administration (GSA).
* Tuberculin Skin Test required annually
* Hospital required vaccinations
* Hospital required competencies

Working Conditions

* Typical office and/or patient care, acute care hospital environment.
* Must be able to travel locally between facilities and within the surrounding community.
* Occasional exposure to minimal physical risk
