Job Description

Job Duties
Provide case management services to HIV positive individuals. Conduct a comprehensive intake and assessment with each client which included demographic information as well as a psychosocial history. Develop a service plan with each client and update as needed. Act as client advocate and assist clients in problem solving. Provide appropriate information regarding referrals, ensuring linkage. Provide intervention and support to client when appropriate. Contact client as required by contract. Maintain accurate and up to date information of each client's chart and document per contract. Input data for specific contracts into data tracking system in a timely manner. Advocate with primary care providers to ensure access to quality treatment and/or care. Attend clinical support and actively participate in multidisciplinary meeting. Create and maintain relationship with clinical staff to ensure appropriate and timely care for clients. Maintain appropriate boundaries with clients. Work closely with Medical Care Coordination team to ensure service plan progress. Assist in development of monthly reports. Provide office coverage as needed. Maintain confidentiality of participants. Build and maintain a contact network of agencies and local community members to expedite linkages for clients. Collaborate with other social service agencies. Participate in Quality Management plan. Other duties as assigned within Venice Family Clinic.

Job Qualifications
Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge. Ability to create and maintain boundaries with clients. Ability to complete required program documentation. Public speaking and leadership ability. Skill in managing multiple deadlines and demands. Ability to follow through on assignments. Ability to work independently and as part of a group. Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality. Ability to work effectively with people of diverse cultural and educational backgrounds. Skill in maintaining cooperative working relationships with staff members and volunteers. Skill in creating flyers and other program-promoting documents. Bilingual in English and Spanish. . B.A. or B.S. in Nursing, Social Work, Counseling, Psychology, Marriage and Family Counseling, Human Services, OR a license as a vocational nurse (LVN).
