Job Duties
Under the direct supervision of the Common Ground Case Manager Supervisor, the Sr. Community Health Program Retention Case Manager provides high quality basic case management for people who are HIV+. Provide a higher level of case management for clients who need additional services. Play an integral role in Medical Care Coordination services. Monitor medical appointments and make follow up calls to ensure client had a positive experience and is linked back into care. Maintain a case load of five case management clients. Utilize motivational interviewing to ensure client stays engaged in medical care. Engage clients who have not been in care for over 7 months. Act as client advocate and assist clients in problem solving. Work with out of care clients to identify and address potential and/or existing barriers to engagement in medical care. Maintain accurate and up to date information of each client's chart and document per contract. Input data for specific contracts into data tracking system in a timely manner. Provide appropriate information regarding referrals, ensuring linkage. Provide intervention and support to client when appropriate. Create and maintain relationship with clinical staff to ensure appropriate and timely care for clients. Work closely with Medical Care Coordination team to ensure service plan progress. Maintain accurate and up to date information of each client's chart and update internal data collection system. Advocate with primary care providers to ensure access to quality treatment and/or care. Attend clinical support and actively participate in multidisciplinary meeting. Maintain appropriate boundaries with clients. Contact client as required by contract. Assist in development of monthly reports. Provide office coverage as needed. Maintain confidentiality of participants Build and maintain a contact network of agencies and local community members to expedite linkages for clients. Collaborate with other social service agencies. Participate in Quality Management plan. Participate in internal and external quality management meetings. Assist in CQI activities as needed. Maintain a general sense of current data for quick reference. Assist direct supervisor in ensuring data is accurate. Assist direct supervisor in reporting requirement for specific contracts.

Job Qualifications
Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge. Ability to create and maintain boundaries with clients. Ability to complete required program documentation. Public speaking and leadership ability. Skill in managing multiple deadlines and demands. Ability to follow through on assignments. Ability to work independently and as part of a group. Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality. Ability to work effectively with people of diverse cultural and educational backgrounds. Skill in maintaining cooperative working relationships with staff members and volunteers. Skill in creating flyers and other program-promoting documents. Bilingual in English/Spanish. B.A or B.S. in Public Health, Social Work, Psychology, Counseling, Nursing, Human Services, Health Education OR a minimum of three years conducting outreach to vulnerable populations with the aim of improving health outcomes.
