Job Description

Job Duties
This position provides administrative and clinical leadership to the Care Coordination and Social Work Departments, as well as assuming a leadership role on various interdisciplinary committees and taskforces especially those focused on the coordination of care transitions both to and from UCLA Health. This senior director role requires both local management of care coordination and system-wide leadership in patient care management, which includes assuring prompt patient access to UCLA Health services and liaison with payers for purposes of utilization review. Administrative leadership on interdisciplinary committees and task forces entails serving as a catalyst, facilitator and coordinator for interdisciplinary systems improvements as they relate to patients and staff. This position supports the organizational goals and ensures that departmental goals are in full alignment. The System Director is accountable for the operational structure, financial performance and strategic initiatives that include Care Coordination, Social Work, Utilization Review, Patient Throughput and Transition Management. The System Director reflects the mission, vision and values of UCLA Health.

Job Qualifications
Must be a graduate of an accredited school of Nursing or Social Work, currently licensed in California or eligible for licensure Holds a MS, MA, MBA or Master's degree in a related field. Doctoral preparation preferred Senior Director of Care Coordination in an acute hospital setting required Minimum of 10 years experience at the Department Director level; preferably at a system level Demonstrated progressive leadership experience and success in managing and leading innovative programs, building alliances with community providers to improve Care Coordination and Transition Planning Accredited Case Management (ACM) or Case Management Certification (CMS/CCM) Interpersonal skills necessary to interact with Medical staff and Hospital management and Administrative staff in a variety of situations Analytical skills necessary to measure the effectiveness of Care Coordination, Social Work, Utilization Review Excellent verbal and written skills
