Job Duties
Incumbent is responsible for coordinating activities surrounding the patient centered medical home for high risk, high utilizer patients. Core activities include identification of high risk patients, transitions of care management, health system navigation, patient advocacy and linkage to resources, and longitudinal care delivery. Comprehensive Care Coordinators are paraprofessionals who are embedded in the outpatient clinics who work as an extension of the trusted clinician to manage high urgency tasks and projects for high risk, high utilizer patients to avoid unnecessary ED visits and hospitalizations. Work is assigned in two main ways - through physician referral and through reporting registries. Comprehensive Care Coordinators report to the Managers of Comprehensive Care Coordination and work under the general supervision of the Faculty Practice Group Chief Medical Officer (CMO), Medical Director for Accountable Care, and Director of Comprehensive Care Coordination for Population Health Management and receive guidance from the Clinical Advisors (RN and LCSW). They must work collaboratively with the clinic physicians and managers to promote patient care.

Job Qualifications
Proficiency in Microsoft Office Suite. Skill in reading and understanding documents written in standard English text for informational communication, action and distribution. Writing skills to compose, edit and proofread routine clinical and business correspondence using appropriate English grammar, syntax, spelling and punctuation. Accuracy in message relay. Oral communication skills to speak with individuals of varying backgrounds to obtain and convey specific information. Proven ability to establish and maintain cooperative working relationships with physicians, staff, patients, and administrative personnel. Proven ability to critically think through complex situations by independently analyzing information or problems, identifying relevant factors, developing trends, and outlining/prioritizing options to implement and monitor solutions. Proven ability to perform basic mathematical calculations quickly and accurately. Demonstrated skill in independently organizing and prioritizing workload in order to meet deadlines in light of changing, heavy workload and competing requirements. Proven ability to perform job duties quickly and multitask in a detailed and accurate manner in a fast-paced environment. Demonstrated skill in executing tasks despite frequent interruptions and/or distractions. Proven ability to maintain helpful, caring and courteous relationships with leadership, patients, co-workers, and physicians in high-stress/high-urgency situations. Experience encouraging patients to follow-through on complex care plans. Working knowledge of community resources. Experience collaborating with leadership and escalating issues in a timely manner for communal decision-making and feedback. Ability to work occasional overtime, as needed. Experience working in an ambulatory setting and working knowledge of insurance plans/payors. Knowledge of Medical Terminology. Ability to speak conversational Spanish preferred.
