Job Description

Job Duties
Under the direct supervision of the Director of Common Ground, outreach to most vulnerable and hard to reach population in west Los Angeles and the surrounding areas. Build relationships with the target population and link them to syringe exchange, HIV/Hep C/STD testing, and substance abuse treatment. Act as a liason for those that connect to care.Conduct HIV and Hepatitis C testing and counseling on site and in the community. Ensure all documentation meets the funding requirements. Create and maintain relationships with various "gate keepers" within surrounding communities to provide information regarding programs. Seek clients with unknown HIV/Hep C status, HIV+ patients who are lost from care, clients who use syringes and clients who desire substance abuse treatment and link back to care at VFC. Provide referrals to services offered by agency. Maintain appropriate boundaries with clients. Review and complete data collection forms. Provide HIV/Hep C testing. Conduct walk-in testing on site Compile data for monthly report. Ensure program meets all contract requirements. Ensure newly diagnosed HIV+ and/or Hep C+ clients are referred and linked to medical services. Provide clean syringes to clients at certified locations. Dispose of dirty syringes in appropriate manner. Educate client on overdose prevention, wound care, prevention education and referrals to various services. Provide referrals to Hepatitis C, STD and HIV testing. Support and develop culturally appropriate education and outreach materials for programs. Maintain confidentiality of participants. Document according to funding requirements. Build and maintain a contact network of agencies and local community members to expand prevention program services. Participate in annual audits. Participate in Quality Management plan. Other duties as assigned within Venice Family Clinic.

Job Qualifications
Knowledge of HIV/AIDS, including Antibody testing, community resources, Hepatitis C and STD knowledge, harm reduction, syringe exchange. Skill in database management and data entry. Ability to complete required program documentation. Public speaking and leadership ability. Skill in providing individualized HIV health education, HIV testing and Counseling, Hepatitis C education, Hepatitis C testing and counseling, overdose prevention, medical and substance abuse referral, and abscess education. Ability to follow through on assignments. Ability to work independently and as part of group. Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality. Ability to work effectively with people of diverse cultural and educational backgrounds. Skill in maintaining cooperative working relationships with staff members and volunteers. Skill in creating flyers and other program-promoting documents. Bilingual in conversational Spanish.
