Job Description

Job Duties
The Case Manager is responsible for high-level assessment and planning for an identified group of patients in the Medical Center including the Emergency Room. Collaborates with the entire medical team to coordinate the delivery of comprehensive, efficient, compassionate, cost effective patient care. CM coordinates resource utilization, assesses appropriate of level of care with approved criteria, and evaluates services provided. Per Diem position will fulfill weekend coverage, especially Sundays.

Job Qualifications
California RN license required. BSN is required, MSN preferred; Case Management experience a plus 3 years' acute hospital experience, emergency room preferred or equivalent in education/training and experience Team Oriented, leadership skills High level of flexibility and collaborative approach to work Self-directed, confident and assertive. Creative problem- solving skills in systems planning, patient care delivery in a high volume work environment. Effective verbal and written communications with multiple types of disciplines and populations. Computer proficiency a must.
