Job Description

Minimum: $53.59 - Maximum: $68.89

JOB SUMMARY

Reports to the Ambulatory Care Clinical Pharmacy Manager. Implements and conducts clinical ambulatory pharmacy services within Saint Joseph Health System and is driven to meet and exceed customer expectations. Holds self and department accountable for achieving the highest level of quality of care, customer service, and employee satisfaction. Recognizes the contribution for desired results. Assists the director in selecting, developing, and maintaining a team of proficient staff.

JOB DUTIES

1. Actively demonstrates the organization's mission and core values, and conducts oneself at all times in a manner consistent with these values.

2. Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.

3. Facilitates the development, implementation, monitoring of adherence to, and administering departmental policies and procedures. Interprets policies and procedures to associates.

4. Recommends and implements agreed-upon long and short-term departmental goals and objectives in order to enhance department services. These goals are aligned with corporate goals and objectives.

5. Assists associates in the development, administration, and improvement of all ambulatory pharmacy programs and services. Independent development of drug use evaluations, ADR reporting, Clinic Development and Management, and Creation of Business Plans.

6. Actively participates in the Pharmacy Residency Programs as well as Medical Residency Programs, in addition to precepting pharmacy students.

7. Performs the duties and responsibilities of the position of pharmacist as dictated by staffing situations or workload. Including but not limited to: Transitions of Care, Anticoagulation, Diabetes Education and Management, Asthma Education and Management, Medicare Annual Wellness Visits, MTM Visits, CHF Education and Management, or other ambulatory care pharmacist managed clinics that come to fruition.

8. Communicates effectively, tactfully, continuously, clearly and positively with the entire scope of customers, visitors, and staff. Obtains clarification of medication orders. Supply information regarding the safe and effective use of medications to associates, physicians, patients or other groups as needed.

9. Interviews and trains associates and pharmacy residency candidates. Has involvement with associates to improve clinical quality, customer satisfaction, and internal employee satisfaction.

10. Independently reviews and makes recommendations regarding medication therapy management. Attends rounds or patient care conferences as required. Evaluates utilization of resources and cost effectiveness of therapy decisions.

11. Maintains current knowledge regarding pharmacy, ambulatory care pharmacy, and healthcare related topics.

12. Performs other duties consistent with purpose of job as directed.

JOB SPECIFICATIONS AND CORE COMPETENCIES

Education: Requires the knowledge of theories, principles, and concepts acquired through the completion of the degree of a Doctorate of Pharmacy.

Licensure: Must hold a valid Indiana pharmacy license or be a graduate of an accredited school or college of pharmacy and licensed in the state of Indiana as a pharmacy intern.

Experience: Candidate has PGY-1 and PGY-2 Residency Training with emphasis on Ambulatory Care. In lieu of residency training, candidates have five years or greater pharmacist experience in the ambulatory care setting. BCPS or BCACP Certification preferred.
