Job Description


Position Title: Outreach Coordinator – Council on Aging
Statement of Duties
Employee performs responsible administrative and advisory work in providing resource and
referral information to seniors in the community, and their families. Work includes assessing the
needs of seniors and recommending services and benefits available to them, supervising
volunteers, developing and administrating programs, and assisting in the coordination and
implementation of activities at the Senior Center.


Supervision
Employee works under the general direction of the Director of Elder Services. Employee plans,
prioritizes, and performs work independently, in accordance with standard practices and previous
training. Employee is expected to solve most problems of detail or unusual situations by
adapting methods or interpreting instructions to resolve the particular problem. Instructions for
new assignments or special projects usually consist of statements of desired objectives,
deadlines, and priorities. Unusual situations are discussed with supervisor. Work is generally
reviewed for appropriateness of actions or decisions, and conformance with Department policies
and other regulations. Employee has access to confidential information of seniors.
Employee is responsible for the supervision of approximately six volunteers; some are located in
separate areas than the supervisor. Employee is responsible for the evaluation of these
volunteers. Workload is relatively stable. Any increases in workload can usually be planned for
in advance.

Job Responsibilities
Work requires examining, analyzing, and evaluating facts and circumstances surrounding
individual problems, situations, or transactions, and determining actions to be taken within the
limits of standard or accepted practices. Guidelines include a large body of policies, practices,
and precedents that may be complex at times. Employee is expected to use sound judgment to
analyze specific situations to determine appropriate actions. Employee is expected to weigh
efficiency and relative priorities in conjunction with procedural concerns in decision-making.
Work requires understanding, interpreting, and applying complex federal, state, and local
regulations. Work consists of the practical application of a variety of concepts, practices, and
specialized techniques relating to outreach services.
Errors can result in a delay of service, legal repercussions, or monetary loss.
Employee has frequent contact with the public, especially the senior population and their
families, other town departments, social service agencies, health care professionals, lawyers,
housing, and volunteers for the purpose of coordinating services; giving or receiving information
and assistance; resolving problems; coordinating work for the purpose of providing resource and
referral information; responding to inquiries; giving or receiving information; responding to
Hull, Massachusetts requests or complaints; providing explanations. Contact usually occurs in person, in writing, Email or on the phone.


Position Functions
The essential functions or duties listed below are intended only as illustration of the various

Essential Functions
1. Assists seniors and their families with accessing benefits and resources; provides
information and referral.
2. Conducts telephone consultations, on site meetings, or in-home visits to assess the needs
of individual seniors and makes referrals appropriate to the situation.
3. Reports cases of suspected elder abuse (physical, emotional, financial and sexual) to the
designated protective service agency. Reports seniors who may considered an “elder at
risk” to the designated protective service agency.

4. Serves as a mandated reporter in accordance with Chapter 651 CMR 5.00, the Elder
Abuse Reporting and Protective Services Program, and is subject to fine for failure to
report suspected abuse and/or suspected “at risk elders”.
5. Provides case management when necessary to help coordinate the services of other
involved agencies such as South Shore Elder Services, Visiting Nurse Association
(VNA), and other home health agencies. Oftentimes such cases are complex in nature
and involve hoarding, abuse, or diminished capacity.
6. Coordinates and works with Board of Health, Police, Fire and Veterans’ Services to
reach resolutions regarding issues encountered by all involved around a particular
individual.
7. Assists seniors and/or others acting on their behalf with the completion of benefits
applications for food stamps, Low Income Energy Assistance Program, property tax
deferral, Mass Health, and other benefits as are appropriate.
8. Advocates for seniors with local, state and federal agencies around the receipt of
benefits; reinstatement of Medigap (Medicare supplemental insurance); approval of
benefits applications.
9. Completes applications for the Low Income Energy Assistance Program (fuel
assistance) for all Hull residents regardless of age.

10. Develops and maintains relationships with outside agencies and professionals such as
VNA, home health agencies, legal services.
11. Maintains an inventory of resources designed to serve seniors..
12. Oversees the SHINE (Serving Health Information Needs of Everyone) Program and
supervises the SHINE Counselors assigned to the program. Also provides direct
counseling around health insurance benefits in the absence of a SHINE Counselor.
13. Assists with the planning, implementation, and supervision of activities at the Senior
Center.
14. Attends professional meetings and workshops in order to stay current with changes in
regulations that affect services and benefits available to elders including, but not limited
to health insurance programs, food stamps, and home care services.
15. Maintains accurate and current records of client information through the Department’s
database system.
16. Writes outreach page for the newsletter.
17. Maintains confidentiality of all persons served.
18. Performs other related duties as required, directed or as the situation dictates.
Physical and Mental Requirements
Employee works in a moderately loud senior center and is required to make home visits and may
be exposed to unsanitary living conditions and hostile or confused people. Employee is required
to talk, listen, hear, sit, use hands, stand, walk and assist frail elders. Employee frequently lifts
up to 10 lbs. Normal hearing and vision is required for this position. Equipment operated
includes automobile, wheelchairs, office machines, and computers.


Education and Experience
A candidate for this position should have a Bachelor’s Degree in Human Services, Gerontology
or related field and at least five (5) years of experience in the field of gerontology, working
directly with seniors or equivalent education and experience.
Special Requirements
A valid driver’s license and criminal history background check (CORI) are required as well as a
physical.
