Basic Job Functions:

  • Coordination of individualized plan of care from an interdisciplinary team approach.
  • Reviews and evaluates resident MDS assessments and plan of care; ensures appropriate implementation.
  • Directs resident care plan conferences.
  • Conducts weekly wound rounds with the charge nurse. Ensures weekly measurement, documentation and implementation of appropriate wound intervention as directed by the attending physician.
  • Completes the infection control log sheet in a timely manner with information of all residents identified with infectious disease. This includes a monthly summary and analysis of cases identified.
  • Updates the Department of Health of infections affecting more than one resident and showing trends for appropriate intervention/action.

Job Qualifications:

  • Degree in Nursing required.
  • Registered Nursing with active license in good standing in state of work site.
  • Minimum 2 years of clinical experience. Minimum of 1 year of experience in a long-term care setting required.
  • Prior experience as an MDS coordinator is preferred.
  • Must possess strong written and oral communication skills; able to maintain confidential information.
  • Must be proficient in Microsoft Office.
