SUMMARY:

Accountable for the development, implementation and continuous improvement of the incident reporting, investigation and learning programs, tools, reports and systems for Andeavor. Champions a robust learning culture across the company and ensures event lessons learned are institutionalized to prevent incidents from happening or recurring.

Provides leadership in setting the strategic direction for the investigation methodologies to be used in our businesses. Establishes an investigator community and defines, establishes and manages a sustainable training model for our investigators.

Defines and establishes the ‘learning from incidents strategy’ to foster a culture of learning and continuous improvement. Establishes a learning framework, processes and tools to facilitate the review of applicable internal and external industry insights and lessons learned.

Accountable for defining and embedding metrics to monitor the ‘health’ of the incident reporting, investigation and learning programs.

KEY RESPONSIBILITIES:

  • Develops strategy and plan to build reporting maturity, and investigations and learning capability across Andeavor.
  • Champions a strong incident reporting and learning culture by ensuring our programs and leadership behaviors promote an environment that facilitates learning from incidents.
  • Oversees and ensures effective incident reporting and investigation programs. Develops and maintains corporate standards on incident reporting and investigations to ensure timely reporting and quality investigations and reports that result in meaningful insights and appropriate corrective actions.
  • Determines the appropriate root cause analysis techniques to be used for various investigation levels. Develops and provides training to ensure the organizational capability exists to apply the proper root cause analysis techniques.
  • Develops and runs “learning from incidents” programs, including but not limited to development of High Learning Value Events, cross-functional learning forums, Safety Flashes, etc.
  • Oversees management of incident reporting data system applications. Oversees analyses of the data system application to monitor the health and maturity of our reporting culture and program.
  • Removes any roadblocks to reporting and investigating, e.g. by improving tools, simplifying processes, and focusing on that which promotes learning.
  • Oversees analyses of incident data to identify and communicate trends in incident root causes and risk areas. Influences businesses to address the risk areas.
  • Keeps abreast of and introduces new methods or approaches to investigation and learning, e.g. human reliability or situational awareness analysis.
  • Provides technical expertise and leadership on high level incident investigations as needed by the organization.
  • Ensures incident investigation and reporting programs are in compliance with applicable regulations and company policies and guidelines.
REQUIREMENTS:

EDUCATION

  • Minimum undergraduate degree in engineering, natural or physical science, or other related technical discipline required.

Experience

  • Minimum 10-years engineering and/or industrial EH&S experience in oil/gas industry required.
  • Refining or chemical industry experience preferred.
  • Experience administering environmental, safety and process safety programs preferred.
  • Previous supervisory experience required.

Skills

  • Effective oral and written communication skills.
  • Proficiency with incident investigation and root cause analysis methods such as ABS root cause analysis, Fault Tree, Barrier Analysis, Tap Root, Think Reliability, Why Analysis, etc.
  • Ability to develop and implement corporate wide management systems and programs.
  • Powerful collaborator, able to work with and influence peers, functional and operational leaders.
  • Strategic planning and thinking.
