We are Now Hiring Full - Time Community Mental Health Navigator

Position qualifications: Bachelor’s degree in human services or related field preferred. At least 3 years previous experience in human services related field required. Direct experience working with diverse populations required. Detailed knowledge of Summit County mental health system and community services preferred. Bilingual required. Master’s degree in social work, or related human services preferred.

Job Description: This position will be responsible for helping residents connect to the appropriate level of mental health care in Summit County through general navigation services. This position will additionally provide comprehensive case management and care coordination for high-risk or high-acuity individuals and or individuals with highly complex needs to ensure a consistent and coordinated delivery of services. The goal of the Mental Health Navigator position is to reduce barriers to care, work directly with the client to improve the mental health outcomes through supportive case management, provide access to early intervention, risk reduction, client education and empowerment.  This position’s scope of work does not include providing therapy or treatment.

The Mental Health Navigator will work within the context of a client-centered, recovery-oriented, wraparound approach, in partnership with mental health providers, agencies and physicians to promote: timely access to needed care, comprehension and continuity of care, client empowerment and the enhancement of client and family wellbeing.
