Job Description


Role Purpose:

Interviews patients on admission to collect medication histories and assists with medication reconciliation processes to ensure that any differences between medications taken before admission and medications ordered upon admission are intentional and void of negative patient incident. Assists with the discharge medication reconciliation process at coordination with pharmacists and case managers. Offers discharge prescription service through the retail Pharmacy. Collects appropriate insurance, payment and demographic data for prescription processing.

* Works cooperatively with others and organizes daily workflow to optimize output. o Uses time wisely to promote value-added services. o Prioritizes tasks to complete critical patient care duties first. o Reduces non-essential interruptions to deliver optimal patient care and maximize productivity. o Works independently taking initiative to assess and meet workload demands. o Discusses progress and concerns related to job duties with a pharmacist to ensure that consistent services are provided on time. o Functions as a team player. o Works independently without direct supervision and uses time efficiently.
* Completes medication histories for new admissions. o Identifies patients to be admitted using Electronic Health Record (HER) tools and other sources of information. o Assesses the patient, caregiver, or other appropriate persons to determine their ability to provide accurate medication history information. o Interviews the patient, caregiver, or other identified persons to obtain the medication history. o Determines which records may be used to obtain a medication history when a person is not able to provide the information. o Follows up with pharmacies, nursing homes, family members, or others as needed to obtain complete medication history information. o Determines whether information is accurate and when a pharmacist needs to become involved. o Accurately records the medication history obtained into the hospital's computer system.
* Promotes hospital's retail pharmacy program through offering of discharge prescription services. o Enrolls patients upon admission regarding discharge prescription services. o Coordinates collection of appropriate insurance (if needed) and demographic patient information for prescription processing. o Interacts with other healthcare professionals to coordinate sending of prescriptions, either paper or electronic, to the hospital retail pharmacy. o Coordinates with hospital and retail pharmacy staff for timely processing and filling of prescription(s). o Coordinates bedside delivery of prescription(s). o Communicates appropriately with inpatient and outpatient pharmacists to facilitate discharge counseling.
* Provides service to customers to meet or exceed their needs and expectations. o Investigates discrepancies in the medication histories and other problems and follows up with pharmacists to facilitate resolution of any issues. o Communicates with customers (e.g., patients, caregivers, family members, physicians, nurses, pharmacists, and others) to obtain and communicate accurate information, resolve problems and meet customer needs. o Offers discharge prescription services through the retail pharmacy. o Utilizes telephone skills and etiquette to obtain and provide information when needed. o Communicates progress, needs, and concerns to pharmacists, co-workers, or a coordinator to facilitate problem-solving and follow-up to ensure customer satisfaction.
* Collects and compiles data, statistics and other information needed for quality assurance programs or reports to provide information to assist with decision-making. o Collects data and maintains records for of patient information per policy.

* Participates in the team approach to quality improvement to support the Pharmacy Department's commitment to the philosophy and principles of Continuous Quality Improvement. o Participates in the establishment of area and departmental goals and objectives. o Sets and meets individual goals which contribute to the team's success. o Participates on teams to resolve problems and improve processes. o Participates in peer review activities. o Contributes to team results and customer satisfaction. o Assists other staff members with their job duties when finished with individual work assignments. o Responds positively to internal and external feedback.
* Maintains updated knowledge and skills and contributes to the education of others. o Attends required in-services to meet JCAHO, OSHA, and other departmental requirements. o Assists with the training of new employees, technician students, high school students, and others as needed. o Passes all competencies as mandated by the hospital and pharmacy department. o Maintains annual registration with the Wisconsin Board of Pharmacy
* Provides age/developmentally appropriate care in accordance with Age/Developmental Specific Care Guidelines for the age groups served when communicating with patients and/or their families. o Considers age differences when communicating to patients and visitors, e.g., demonstrating awareness of potential hearing, sight, motor skills and short-term memory for elderly; and language skills for children. o Maintains competence related to age-specific care through continuing education as necessary.


Minimum Qualifications:

* High School diploma
* CPhT Certification through the Pharmacy Technician Certification Board. If not certified on hire, will have a 6 month grace period from start date to certify
* Minimum of two years of Pharmacy Technician experience or enrolled in an accredited school of pharmacy program

Preferred Qualifications:

* College degree

Department Specific:

Must possess a thorough knowledge of medical and pharmacy terminology. Strong computer skills for entering and retrieving information from necessary pharmacy computer programs, and demonstrated proficiency with Word and Excel programs. Excellent customer service and communication skills for interacting professionally with pharmacy team members and other clinical staff members. Strong problem solving and organizational skills for working independently, meeting deadlines, and establishing priorities in a variety of assignment areas. Must be able to provide instruction and leadership.

Physical Requirements:

Employees in this position are, or can be, required to possess the following physical capabilities:

* Lifting, carrying, pushing and/or pulling up to 10 lbs.
* Stooping, kneeling and/or crouching daily.
* Standing or walking more than 8 hours a day.
* Reaching, grasping, fingering, and/or feeling more than 8 hours a day.
