Our Rehab and Nursing Centers have been providing services in West Michigan for a 20+ years. Our residents range in age and diversity. At both of our locations on Fuller Ave. and Kalamazoo Ave. we help residents live a healthy and active life to the greatest level possible.

* We offer positions as a Licensed Practical Nurses and Registered Nurses in both facilities.
* Your experience and licensure will allow us to discuss competitive pay and an outstanding benefits package.
* Shifts at each facility range from days (1st shift), evenings (2nd shift mostly 8 hours), and nights (3rd shift). Each shift has unique shift differentials attached to weekday and weekends rotations.
* We also offer flexible positions that accommodate your lifestyle.
* Resource (PRN) positions, Part-time (with benefits), and Full-time roles.

Fuller Avenue Facility (750 Fuller Ave. NE Grand Rapids, MI 49503)

* This facility has been a cornerstone to our Grand Rapids Community for years. Fuller Avenue is a staple to the Spectrum Health's continuum of services.
* Fuller has 5 Floors that provides care for Long Term Care and Post-Acute Rehabilitation patients.
* Each Floor has between 50-60 active beds.
* Nurse to patient ratios vary depending on the shift and the population you'll be serving.

Kalamazoo Avenue Facility (4118 Kalamazoo Ave. Kentwood, MI 49508)

* This two-floor facility was newly constructed in 2016 replacing the previous building (still located behind).
* It has a 76 bed Post-Acute Rehabilitation unit and an 88 bed Long Term Unit.
* Each floor has designated units that focus on patient specialties such as: General Rehab, Neuro, Traumatic Brain Injuries, Cancer, etc.

LPN Position Summary:

Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.

RN Position Summary:

Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment, diagnosis, outcomes identification, planning, implementation, and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice, culturally congruent practice, communication, collaboration, leadership, education, evidence-based practice and research, quality of practice, professional practice evaluation, resource utilization and environmental health. Strives to achieve optimal outcomes.

RN:

Basic Qualifications:

Education - Associate's Degree or equivalent in graduate of an approved nursing program

Experience - Successful completion of a competency based orientation program

Licenses - Registered Nursing license in MI (RN)

Driver's License - MI - currently unrestricted, automobile insurance, and available automobile for NRS Home Care

Cardiac Pulmonary Resuscitation (CPR) within 180 Days of hire

Basic Life Support (BLS) within 90 days of hire

Advanced Cardiac Life Support (ACLS) within 1 year of hire

Preferred Qualifications:

BSN - Bachelor's Degree in Nursing

Long Term Care Experience

LPN:

Basic Qualifications:

Education - High School Diploma or equivalent

Associate's Degree or equivalent -graduate of an approved nursing program

Successful completion of competency based orientation program

Licenses - Licensed Practical Nurse in MI (LPN) State of Michigan

Drivers License - MI (MI DR LIC) State of Michigan for NRS Home Care

Basic Life Support (BLS) within 90 Days of hire

Certified Brain Injury Specialist (CBIS) Certification for Continuing Care within 2 years

Preferred Qualifications:

Experience - Healthcare experience
