Seasons Hospice & Palliative Care is seeking a Hospice Aide Coordinator to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care. The Hospice Aide Coordinator will coordinate the scheduling of Hospice Aide visits with the manager, clinical team (RN) supervisors to meet and maintain the care plan requirements for patients / families and the staffing needs of the program.


SHPC recognizes the important role of the Hospice Aide on the hospice team. In programs with a smaller census or times when staffing needs require additional staffing the following duties may be in addition to providing direct patient care for an assigned caseload. This position supports the Hospice Aide by acting as liaison and mentor as well as providing a voice for the Hospice Aide on committees and the management team.

Qualification

*Certified in state(s) practicing as a Nurse’s Aide, or Certified Home Health Aide (CHHA), or Certified Geriatric Nurse’s Aide (CGNA).

*Board Certified Hospice and Palliative Nursing Assistant (CHPNA).

*At least two years’ experience in Hospice care with strong communication and organizational skills.

*Must have reliable automobile, valid driver’s license, and the minimum state required liability auto Insurance.

*A compassionate attitude toward the care of the sick; the ability to read, write and carry out directions, maturity and the ability to deal effectively with the demands of the job.

*Demonstrated commitment to Seasons Hospice’s philosophy of care, values, mission statement and organizational culture.
