Description

Seasons Hospice & Palliative Care is seeking a Nurse Practitioner to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.


Under the supervision of the Medical Director or other staff physicians, the Nurse Practitioner will partner with both the Hospice and Palliative care team to direct and facilitate the care and support of patients, families and the physicians who treat patients that are facing chronic illnesses or a life threatening illness. The Nurse Practitioner will join and assist the Physician in providing comprehensive medical care to patients in a variety of settings, including: home, hospitals, nursing homes, and assisted living facilities. As a member of the Palliative Care team the Nurse Practitioner will provide education on Hospice care for physicians, patients/families and others.

Qualification

Nurse Practitioner licensed in state(s) practicing.

Certified by a nationally recognized advanced nursing practice organization.

Current Cardio Pulmonary Resuscitation (CPR) certification.

Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.

Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.

Demonstrated expertise in pain/symptom management and advanced care planning.

Demonstrated commitment to hospice philosophy of care and willingness to serve on-call.

Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance.

Demonstrated commitment to Seasons Hospice's philosophy of care, values, mission statement and organizational culture.
