Must attach proof of graduation from an accredited school in a Certified Occupational Therapy Assistant (COTA) Program. Proof of two years of COTA experience is preferred. Must attach your valid license as a Certified Occupational Therapy Assistant issued by the California Board of Occupational Therapy.
