Job Description

Premier Physicians Centers is Northeast Ohio's leading independent, multi-specialty medical practice.

Our growing group practice encompasses internal and family medicine, medical and surgical specialties, and has comprehensive on-site diagnostic centers conveniently located in the communities we serve in the Cleveland metro area.

The Clinical Social Worker collaborates with providers of health care to meet identified complex and episodic medical and social needs of the patient and to promote the efficient and cost-effective delivery of health services. The Clinical Social Worker is responsible for providing developmentally appropriate clinical counseling and mental health services to patients within an integrated healthcare and wellness model.


  • Master's degree in Clinical Social Work with independent license LISW or LISW-S or LCSW in the state of Ohio.
  • At least two years of professional experience (post-licensure) working with individuals with behavioral health, physical and behavioral disabilities, medically ill; prior behavioral health environment experience is required.
  • Knowledge of DSM diagnostic criteria and treatment planning, self-management support techniques.
  • Apply knowledge of psychiatric medications and ability to evaluate responses.
  • Ability to work independently under general instructions, self-directed and motivated.
  • Excellent verbal and written interviewing and assessment skills.
  • Basic knowledge of major chronic disabling illnesses for persons of all ages, disease processes.
  • Ability to effectively communicate with the disabled and elderly populations and general public, both in person and by phone.
  • Ability to communicate in a collaborative, effective manner with others and to maintain good working relationships; able to establish rapport quickly with a wide range of people.
  • Ability to analyze, evaluate, and implement a reasonable course of action, plan of care, based on available information.
  • Demonstrate ability to work within and coordinate a care team comprised of internal team members and community-based members.
  • Demonstrate the ability to handle stressful situations appropriately. Strong organizational skills and able to multitask while remaining professional, focused, composed and positive.
  • Basic level of experience with Microsoft Office, with the ability to navigate a Windows environment.
  • Work from home opportunities may be available after 12-18 months of employment, determined by business need; Must have distraction-free area in home for home office.
  • Must have access to high speed Internet (DSL/Cable).
  • A personal vehicle, valid State of Ohio motor vehicle operator's license and conformity with insurance coverage limits are required.
