Essential Medication Reconciliation Pharmacy Technician job functions and responsibilities:

* Strong communication skills to effectively interview patients and families to obtain or verify medication histories.

* Contacts retail pharmacies and physician offices to complete or verify medication histories.

* Enters home medication lists into appropriate computer systems.

* Demonstrates proficient skills in using computer systems and paper charts for data retrieval.

* Works closely with emergency department clinical pharmacist and nursing staff.

* Provides clinical care staff with completed admitted medication histories.

* Promptly identifies any medication history issues with clinical pharmacist or staff.

* Communicated effectively with other healthcare professionals for medication history problem resolution, including physicians.

* Unit Inspection Functions in the Emergency Department.

* Generalist functions.

* Train and mentor other medication reconciliation technicians.


* Completion of Pharmacy Technician program (preferred)


* Active New Hampshire State license as a Pharmacy Technician (required)

* National Certification from the Pharmacy Technician Certification Board (PTCB) (required)


* 1-3 years of Pharmacy Technician experience (required)

* Retail Pharmacy experience (preferred)


* Full Time
* 7 days on/7 days off

* Wednesday - Tuesday

* Evening Shift
* 12:30pm - 11:00pm
