PeaceHealth is seeking a Program Coordinator - Spine & Total Joint for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of 8 Hours.

JOB SUMMARY

Responsible for serving as the day to day facilitator within the Total Joint and Spine Program. Will coordinate and supervise the development, implementation, and organization of all elements of both programs. Responsible for coordinating the efforts between providers, administration, staff, volunteers and patients while tracking the program outcomes and identifying areas for improvement. Responsible for patient education, coordination of patient activities and administrative activities within the program.

ESSENTIAL FUNCTIONS

  1. Facilitate program development and oversees the day to day patient management.
  2. Coordinate and lead regularly scheduled meetings effectively with all stakeholders within the Spine Center and Total Joint Replacement Program, such as unit staff, providers, therapy staff, and other key team members.
  3. Develop and maintain standardized systems of care with the surgeons, including pre-op, post-op and discharge orders.
  4. Maintain effective communication with team members and stakeholder departments.  Build successful relationships with various internal and external stakeholders.
  5. Serve as the clinical resource for joint replacement and spine care best practice by educating staff and providers, monitoring patient care in real time, and advocating for necessary elements of care.  Develop and help maintain standardized care plans with the providers and other key clinical areas including pre-op, post-op and discharge orders.
  6. Ensure appropriate pre-op education and preparation for each patient prior to surgery. Responsible for developing and overseeing pre and post op teaching classes.  Coordinate and distribute teaching materials to providers’ offices.  Oversees the development and on-going editing of all program forms and materials.
  7. Responsible for Continuous Quality Improvement (CQI) activities to include, but limited to:  tracking surgical outcomes, patient satisfaction, clinical/customer service standards, patient complications, denied days, hospital-wide survey results, nurse call back results, length of stay and volumes, etc. and other areas as need determines.  Responsible for developing and implementing improvement plans and other patient satisfaction activities. 
  8. Attend and maintain membership on appropriately assigned committees. Chairs the Spine Center Performance Improvement Team and the Total Joint Excellence Committee in partnership with the Unit Managers.
  9. Demonstrate good financial / resource stewardship.  Identify patient amenities and secure necessary resources to administer the services.
  10. Organize and participate in community outreach and educational opportunities regarding spine and joint health.
  11. Performs other duties as assigned.

QUALIFICATIONS (Required unless otherwise stated)

EDUCATION:

  • Graduate of accredited school of nursing or physical therapy required.
  • If nursing background, BSN preferred

EXPERIENCE/TRAINING:

  • Minimum of three years’ experience as a clinical RN or PT is required.
  • Minimum of one year health care supervisory or program coordination experience is preferred.
  • Minimum of two years experience in an inpatient orthopedic/rehabilitation/neuroscience setting is preferred.

LICENSE/CERTIFICATION:

  • Current RN or PT licensure in the state of practice is required.
  • Current BLS (Basic Life Support) certification is required at date of hire. (Not required for PHOR).
  • Orthopedic and or Neuroscience Certification is preferred.
  • Certified in Care Coordination & Transition Management (CCCTM) preferred.

OTHER SKILLS:

  • Demonstrated skill in teaching adults of varying competency level is required. Must be able to design and implement instructional modules.
  • Excellent verbal and written skills are required.
  • The ability to interact with all medical center departments and to work independently on projects.
  • The ability to demonstrate a courteous, caring and understanding attitude toward patients, families, co-workers, visitors and providers is required.
  • The ability to maintain confidentiality and effectively communicate with all levels of hospital staff, providers and patients is required.
  • Proficient computer skills including MS Office applications.

SOUTHWEST PHYSICAL/COGNITIVE REQUIREMENTS:

  • Work requires moderate physical exertion from 34 to 65% of the time.
  • Ability to lift objects weighing 30 lbs. or less.
  • Work is performed under normal working conditions with adequate lighting and ventilation.
  • Job duties frequently require intense concentration and attention to detail (34-65% of work time).
