PeaceHealth is seeking a Nurse Practitioner - Family Medicine Integrated Medical Home (part-time) based in the Eugene, OR community.

PeaceHealth’s commitment to behavioral health is reflected in a recently completed state-of-the-art inpatient psychiatric unit. Services now extend to new programs including intensive outpatient and partial hospitalization, and expanded wrap-around services to young adults.

Our dedication to innovation includes participation in cutting edge research, telepsychiatry, and behavioral health-primary care integration. As a result of progressive research-based programs our work with early psychosis has garnered national media attention.

*  The FNP will provide primary care (often including managing behavioral health medications) for patients with complex medical, behavioral health and substance use disorders

*  Individual visits are generally longer than in traditional family practice and there are lower productivity expectations built into the model

*  They will be working as part of a team that includes behavioral health providers, MA, nurse (and yoga instructors now too)

*  The team reviews patients’ progress through treatment weekly, with in-depth case reviews of complex cases monthly
