PeaceHealth is seeking a Care Management/Behavioral Health Social Work Lead for a 1.0 full time equivalent Exempt position. This position will be working Day shift with shift durations of 8 Hours.

JOB SUMMARY

Responsible for coordinating the day-to-day functions for assigned area. Assists with ensuring that departmental goals are achieved, timelines met and service excellence provided in all aspects of work. Subject matter expert.

Coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, mental health assessments and coordination of services to provide continuity of care.

ESSENTIAL FUNCTIONS

  1. Assigns work; plans and manages priorities; provides technical assistance; oversees staff schedules; monitors quality of work; monitors team projects; mentors staff; provides constructive feedback; oversees staff training; ensures quality improvement; provides leadership feedback on staff performance; and assists with goal setting for team, which includes Care Manager/Medical Social Worker..
  2. Completes appropriate intake documentation, provides ongoing assessment, develops treatment plan in collaboration with the multi-disciplinary treatment team.  Manages multiple complicated cases, maintains current knowledge of ever-changing community resources, monitors patient needs and progress, and develops aftercare plans.  Maintains documentation on patient throughout hospitalization.
  3. Provides risk assesments (danger to self, others and/or grave disability) to all patients seen and determines need for continued hospitalization or disposition to community services.  Works in collaboration with the attending provider and the medical care team. 
  4. Assists in determining patient level of need and makes recommendations to the care team regarding need for psychiatric consultation, including possible need for medication management.
  5. Integrates the overlapping roles of, consultant, quality improvement facilitator, and clinical educator. 
  6. Conducts community staffings in coordination with the physician and treatment team.
  7. Ensures compliance with current regulatory standands and privacy and confidentiality laws. Interfaces regularly with legal entities to address treatment needs.
  8. Maintains current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, religion and utilizes appropriate communication skills.
  9. Provides information, education and direction in response to community requests.
  10. Regularly accesses pharmaceutical and community resources to provide patients with needed psychiatric medications.
  11. Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA’s.  Must maintain current licensure through continuing education. 
  12. Performs other duties as assigned.

QUALIFICATIONS (required unless otherwise noted)

EDUCATION:

  • Master’s Degree in Social Work from an accredited graduate school of social work.

EXPERIENCE/TRAINING:

  • Two or more years of clinical experience with designated population; hospital, emergency room, chemical dependency, or community agency experience preferred.
  • Must attend PROACT (crisis management) training provided by hospital.
  • Knowledge of community resources, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care
  • Previous leadership experience preferred.

LICENSE/CERTIFICATION:

  • Licensed Clinical Independent Social Worker (LICSW).
  • CCM or working towards, preferred.

OTHER SKILLS:

  • Demonstrated medical and mental health clinical skills to provide clinical supervision and to function as a LIP
  • Demonstrated commitment to promotion of social work values, clinical excellence, and customer satisfaction
  • Familiarity with medical terminology and conditions as these impact discharge needs
  • Excellent verbal and written communication skills
  • Must possess superior ability to work collaboratively within a team.
  • Must possess excellent organizational skills.
  • Ability to document on electronic record

ST. JOSEPH PHYSICAL/COGNITIVE REQUIREMENTS

  • Work requires normal amounts of physical effort at least 66% of the time.
  • Ability to lift objects weighing 30 lbs. or less.
  • Work is performed under normal working conditions with adequate lighting and ventilation. Reasonably anticipated exposure to blood and body fluids.
  • Duties require the ability to concentrate or pay close attention to detail for more than 66% of the work time.
