Job Description

General Summary/Overview: The Community Care Team Intern works closely with the Community Care Team. Under the direction of the team managers, the intern will meet with members of the Community Care Team to gather information about the program and develop a resource guide to be used to educate the broader Pentucket/PCPO/Partners community and potential new employees about the Community Care program. The Community Care Team Intern is responsible for supporting the team and working on special projects assigned by the team managers.

Knowledge/Education:

* Must have a high school diploma or GED


* Must be enrolled in a Bachelor's degree program and have completed 2nd year , majoring in either Psychology, Marketing or similar major


* Must have good working knowledge of computers.

* Must have excellent communication skills. Must have the ability to present information in a clear and concise manner.


* Must have the ability to maintain confidentiality of sensitive information.


* Must be very detail orientated and have excellent organizational skills.


* Must have the ability to multi-task.


* Must have excellent customer service skills.


* Must convey patience, kindness, and empathy for all patients.



Physical Demands:

Requires prolonged sitting, some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment. Requires normal range of hearing and eyesight to record, prepare and communicate appropriate messages.

Environmental/Working Conditions: Work is performed in a fast-paced office environment. There is contant contact with the public, clinic and office support staff. Work may be stressful at times because of high volume.
