Job Description

SIGN ON Bonus eligibility - 2 years of RN experience - current OSF Mission Partners and new grad RNs are not eligible

The Skilled Support Nurse is responsible for the Hospice and Home Health patient visits required after normal business hours. Provides patient-centric triage and clinical care that results in clinical excellence for both Home Health and Hospice patients. Analyzes patient needs, plans and prioritizing care needs for patients of all ages. Skilled to work as a telephone triage nurse and provide support as needed. Ensures quality care is provided through home visits and in the After Hours Triage Department for all entities within the OSF Healthcare Home Care Divisions.

REQUIRED QUALIFICATIONS:

Current licensure by the State Board of Nursing in Illinois and Michigan license can be obtained after hire typically within 8 weeks.

OSF Healthcare Nursing Division requires that all employees beginning employment as a Registered Nurse on or after October 1, 2015 are required to earn a BSN degree within six years from Date of Hire and proof of enrollment within your first 18 months of employment.

Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.

Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.

Maintains a valid driver's license.

Electronic Medical Record and Microsoft application basic proficiency

CPR required prior to patient contact

Analytical and critical thinking skills in order to triage issues/concerns of patients, and make decisions in order to ensure patient needs are cared for.

Excellent oral and written communication skills in order to provide care for the patient in a call center setting.

PREFERRED QUALIFICATIONS:

BSN

Experience as an RN in a healthcare environment

Home Health or Hospice experience
