Serves as clinical expert liaison among the Departments of Cardiology, Medicine,Emergency Medicine, Nursing and Discharge Planning as it pertains to the care of core measure patients. Prepares reports and statistical analysis for the department of Nursing and team conferences. Acts as a resource representative by providing information and facilitating services. Assists in the clinical evaluation and documentation standards of patients with a core measure diagnosis.
1. Implements disease-specific tools for core measure diagnosis, clinical documentation, data abstraction and monitoring of core measure data.
2. Provides ongoing consultation and education to Emergency Department, Nursing and medical staff with respect to the application of continuous quality improvement principles, practices and clinical core measures.
3.Explains and applies accepted methodologies and assesses results.
4.Assists in the development of the hospital/system-wide heart failure initiatives and grant project.
5.Collaborates with management in the preparation of accreditation and regulatory surveys.
6.Reports pertinent data on trends and issues, etc to Hospital-wide Performance Improvement Coordinating Group (PICG). Participates in Health System committee and participates in other committees as necessary.
7.Educates patients/families in the health-illness continuum. Works with nursing staff, homecare, physicians, etc. to provide smooth transition back to the community.
8. Performs related duties, as required.
