Job Description

Meritas Health, a wholly-owned subsidiary of North Kansas City Hospital, seeks and experienced Registered Nurse for the F/T position of RN Case Manager Care Coordinator.

RN will oversee the clinical aspects of our Patient Centered Medical Home and Population Management projects.  This includes devleoping and monitoring care management, the care coordination processes and providing support to the primary care teams.  Position also includes identifying high acuity patients and working directly with them to ensure care is coordinated efficiently and effectivley.  The position may involve paitient triage, but typically not direct patient care.

Required License/Experience:
*State Board recognized nursing education
*State of Missouri Registered Nursing license
*Minimum of three (3) years experience as a licensed Registered Nurse.
*Primary Care experience is highly desirable, but not mandatory.

Required Skills:
*Position requries excellent communication skills at all staff levels and the ability to educate, facilitate, organize and lead discussions. 
*Candidates must demonstrate experience communicating with physicians comfortably and confidently.
*Proficient computer skills; including Microsoft Excel and electronic medical records is mandatory.
*Experience eith eClinicalWorks is a plus.
