The Certified Nursing Assistant (CNA) assists with providing total personal care to the residents in a safe and efficient manner. Accepts direction and supervision from an RN or LPN. Follows established regulations mandated by the state and policies and procedures of Mission Health.


Major job functions include: assisting with providing activities of daily living (ADL's) to residents (including washing, bathing, dressing, feeding, grooming, walking, transferring, skin care, toileting, oral care and catheter care); performing care as defined by resident plan of care/CNA assignment card; visiting with residents; reposition as required; answers call lights; passes nourishments and water to residents; records intake and output; gets report from LPN/RN; observes residents and reports change in condition/attitude/reaction/appetite/behavior to team leader; reports vulnerable adult concerns; takes temperature/pulse/respiration/weight; conducts range of motion as needed; follows facility policy and procedures regarding safety/sanitation/infection control; maintains continuing education requirements.


Must have completed a state approved Nursing Assistant course and have current State certification/registration to practice. Must be responsible to meet the needs of our residents in a safe and caring manner. Follows established policies and procedures and uphold the Resident Bill of Rights. Must be able to organize time and prioritize resident care needs. Exercises confidentiality to ensure compliance with all HIPAA regulations.

Mission Neighborhood Health Center (MNHC) is a 39 year old comprehensive community-based health center [CHC], a Federally Qualified Health Center (FQHC), and a cornerstone in the Mission community. MNHC is a well known San Francisco local leader in advocacy efforts for culturally and linguistically competent health and human services for Latinos in San Francisco and the greater Bay Area. MNHC serves over 13,000 unduplicated patients and renders over 66,000 health care service visits annually. MNHC retains a rich legacy as the first neighborhood health center in San Francisco and the first where Spanish-speaking families could, and still do receive care in their native language from a group of compassionate professionals who are linguistically competent and culturally sensitive. MNHC guarantees families and individuals a "medical home" and provides a one-stop comprehensive, affordable and available health care setting. MNHC works with individuals and families over time and directs efforts affecting long term positive health outcomes within the patient population and community. MNHC denies no patient service based on an inability to pay. Medicare, Medi-Cal, Private Insurance and patient pay are accepted as payment for services. Patients without public or private insurance are charged according to a sliding fee scale based on Federal Poverty Guidelines. Approximately 52% of all care MNHC provides is subsidized through public and private grants and donations. MNHC maintains four (4) active prevention and comprehensive care locations providing ten (10) distinct population service focuses. MNHC is a full service CHC offering Pharmacy, Laboratory and X-Ray. MNHC is an informal training cornerstone for a variety of health professionals: clinicians, nurses, interdisciplinary providers, counselors and clerical clinical support staff.
