Description of Work

SSI/SSDI Outreach, Access, and Recovery (SOAR) is a model that helps individuals experiencing or at risk of homelessness who have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder apply for Social Security disability benefits.

Position Summary

This position completes SOAR applications with people experiencing homelessness and meet qualifying medical and/or mental health criteria. In addition, this position is responsible for supervision and support of the Master of Social Work SOAR Internship program including attending to all graduate student internship documentation as needed by ECHO Intern to fulfill academic duties. This position is responsible for managing a case load of approx. 5 individuals as well as covering intern caseloads as needed. This position will attend supervisory meetings and provide supervision and support to staff daily. This support often includes helping with client/provider issues, as well as crisis prevention involving clients.

Essential SOAR Liaison Functions

* Develop and maintain a working in-depth knowledge of Austin Travis County area homeless a supportive service program eligibility and service criteria
* Must be able to work in the community and meet individuals experiencing homelessness where they are, including in the woods, under bridges, and in abandoned houses.
* Complete SOAR training
* Maintain and track SOAR referrals and manage case load of 5 clients
* Work with community partners through team meetings, outreach, and referrals to facilitate successful client engagement and SOAR program completion
* Complete interviews with individuals to gather information to complete SSI/SSDI applications
* Gather medical records and other information to complete SSI/SSDI applications
* Write SOAR Medical Summary Reports for individual applications
* Accompany individuals to appointments at the Social Security Administration
* Coordinate visits to medical doctors, psychiatrists, and other specialists to obtain evidence for the application
* Coordinate case management services with partners and help with providing case management services to individuals when needed
* Gain familiarity with Social Security Administration's Listings of Impairments and working knowledge of SSA and DDS practice and policies
* Work closely with Outreach and Navigation specialists and Health Care Facility Liaison
* Manage list of target population clients independently
* Maintain a working in-depth knowledge of Austin Travis County area homeless and/or homeless prevention supportive service program eligibility and service criteria
* Frequently interact with people with diverse racial, cultural, and economic backgrounds, gender identity, and sexual identity and treat all community members with dignity and respect. Able to handle client crisis.
* Become familiar with the housing and support programs currently participating in Coordinated Entry process and primary contact for hospitals and other admitting facilities to discuss specific patient cases and determine eligibility for admittance to program
* Maintain documentation in HMIS

Intern Supervisor Functions

* Supervise MSW Interns, oversee SOAR caseloads, and cover caseloads as needed
* Assign SOAR referrals to intern staff once received from supervisor
* Attend client case staffing and communicate updates with SOAR Intern team
* Assist with intern caseload client transportation as needed
* Assist the SOAR team with administrative tasks as needed
* Report SOAR Metrics and use data to drive improvement
* Travel for the purpose of meeting with clients, training, collaborating with stakeholders, or off-site personnel/management
* Ensure intern staff are entering data in HMIS in real time and monitor data quality

Supervisory Responsibility

The position supervises MSW interns.

Work Environment

Employees are required to use their personal cell phones for business purposes. This position will be mobile including the use of personal vehicle. The position will have a stationary space within ECHO's space at a integrated health care clinic. This position requires travel within Austin/Travis County including transporting clients.


This position requires travel using personal vehicle within Austin/Travis County including providing transportation to clients to assist in taking steps towards housing and accessing healthcare.

Additional Eligibility Qualifications & Physical Demands

* Participation in all staff events
* Timesheet submission bi-weekly
* Frequent travel by foot, car or other means appropriate to engage with people experiencing homelessness
* Overtime when necessary to meet project deadlines
* Must be able to adjust to the environment of the target population including making visits to homeless encampments shelters and personal living environments when needed
* Sitting for extended periods of time
* Dexterity of hands and fingers to operate a computer keyboard, mouse, and other devices and objects
* Physically able to participate in training sessions, presentations, and meetings

Position Type and Expected Hours of Works

This is a full-time position working 40 hours a week; typical workdays are Monday through Friday. Occasional evening and weekend work may be required as job duties demand.

Required Education and Experience

* Completion of Master's degree program in Social Work or closely related field.
* At least two years post graduate school experience working directly with people in crisis, homelessness and/or very low-income people. Relevant experience may be considered as a substitute for education.

Preferred Education and Experience

* Bi-lingual (English and Spanish) strongly preferred.
* Experience supervising staff working in direct client services
