JOB DESCRIPTION

PERSONAL SERVICE COORDINATOR/THERAPIST

Department: Transition Age Youth

Reports To: TAY Antelope Valley Program Manager

FLSA Status: Exempt

Category: Direct Service

Summary: Serves as the Primary Support and Case Manager for young adults with mental illness by assisting them to live independently in their chosen community. Provides Licensed-Dependent Mental Health Services to members as needed. Provides clinical consultation and support to MHA-AV Transition Age Youth Staff and Interns. Provides a broad range of clinical training for staff. Responsible for clinical supervision, when indicated.

* Primary responsibilities of the PSC/Therapist include assessing / establishing medical necessity, documenting observed / reported symptoms, determining diagnosis / functional impairments, seek clinical consultation to determine medical necessity.
* Completing Adult Full Assessments and Assessment Updates, and Recovery / Service plans. This also includes performing functions of case management, utilizing knowledge of community-based resources and community development skills, supporting individuals to live as independently as possible in the community, emphasizing the importance of physical health, education, employment, spirituality and its relationship to improved mental health and quality of life. The position also requires paperwork completed in accordance with MHALA, LACDMH County Contracted Provider standards and funding source requirements, and documents in HER
* Must be able to develop a Recovery Plan / Service Plan / Client Coordination of Care Plan that identifies Transition Age Youth Program Participants needs and goals and arrange coordination of supportive services on-site and in the community
* Must demonstrate the knowledge, skills and ability to perform service coordination functions of assessing, planning, linking, monitoring, advocating, engaging and providing therapy for TAY Program Participants in all stages of recovery/change
* Must demonstrate the knowledge, skills and ability to perform in vivo support as required to members' homes, hospital emergency room(s) and other settings as needed
* Must demonstrate the knowledge, skills and ability to monitor overall quality of care, the delivery of supportive services, fidelity and adherence to best practices, promising practices and Evidenced-Based Practices (EBP) through participation in Quality assurance, Quality improvement, and case review activities and through tracking and oversight and outcome data
* Must demonstrate the knowledge, skills and ability to review, assess, research, compile, reconcile and produce a wide range of data and reports as needed or required by management, program supervisor, auditors and/or funding sources
* Provides clinical consultation and support as needed and requested by staff. Provides and supervises intakes and initial assessments
* Notifies Program Manager of urgent and non-urgent referrals
* Coordinates movement of people from one doorkeeper to another and consults on and mediates decision making
* Manages phone inquiries dispersing when appropriate and or by responding to the inquiry
* Perform intakes, initial assessments, and other assessments as Department of Mental Health (DMH) contract requires
* Develop a Recovery Plan and/or Personal Service Plan with each member that identifies his/her personal needs and goals

Position requirements:

* Licensed Clinical Social Workers (LCSW) or Associate Social Worker (ASW) or Licensed Marriage and Family Therapist (LMFT) or Marriage and Family Therapy Intern (MFTI), must be registered with the California Board of Behavioral Sciences.
* Must have a LPS designation or be qualified in a timely manner by the LA County Department of Mental Health to write involuntary holds per section 5150 of the California Welfare and Institutions Code.
* Referral/rehabilitation experience preferred.
* Must demonstrate the skills and ability to observe, notice, reflect on and report thoughts, emotions and/or physical sensations experienced when engaged in decision making and interactions with others in the performance of duties and tasks related to the position.
* Must meet target billing and productivity goal(s) within 24 hours
* Must possess the skills and ability tolerate and respond safely to environmental variables and actively engage in processes and practices (i.e. Pro-Act) that enhance the probability of the best possible outcome for everyone
* Must demonstrate a safety first mentality and engage in practices that promote a safe, healthful environment for all program participants, staff and visitors and providing the best care, welfare, safety and security for everyone at the MHALA AV Enrichment Services.
* Must be able to work under the stress of deadlines; have the ability to prioritize, organize, and juggle multiple tasks
* Must be proficient with reading, speaking and writing English
* Must possess basic typing and computer skills and be knowledgeable in computer applications such as Microsoft Word, Excel and Outlook
* Must be team-oriented, self-directed, have good listening and communication skills, the ability to empathize and relate positively to a wide variety of people and dedicated to fulfilling the demands of the position
* Must be able to function independently, analyze, interpret, draw conclusions from varying facts and circumstances and make decisions
* Must exhibit good personal judgment, a non-bias attitude, sensitivity, and discretion when dealing with other individuals
* Must have knowledge of, and the ability to, use public transportation, public assistance programs and community resources
* Must be able to able to work flexible schedule (occasionally work outside normal business hours)
* Must possess a current class C drivers license, a vehicle for transportation, proof of valid automobile insurance, the ability to be ensured by MHALA liability insurance are conditions of employment
* All billing and supporting documentation shall be entered into MHALAs EHR (electronic health record system) within 24 hours from the date of service delivery
* All required data collection documents (as applicable to your program; MORS, Determinants, KEC, IHIMS, HMIS, etc.) are submitted don or before due date
* TB clearance and re-testing every year are conditions of employment
