Under general supervision of the Program Director of Inpatient Rehabilitation, the Clinical Liaison is responsible for performing pre-admission screenings for Inpatient Rehabilitation patients. The Clinical Liaison assists with the admission process and program development by planning, organizing, and executing community education/marketing activities. The Clinical Liaison may also be responsible for patient care in accordance with established policies and procedures of Medical Center of Lewisville, and standards of practice according to the Clinical Liaison's specific clinical discipline. In collaboration with the medical staff, patients, patient's family and other members of the health care team, the Clinical Liaison is responsible for utilizing a holistic and interdisciplinary approach to coordinate the admission of all patients. In addition, the Clinical Liaison may also be responsible for assessing needs of patients (and in collaboration with Rehabilitation Services), planning interventions and evaluating outcomes of care. The Clinical Liaison may also provide direct care within their scope and practice and delegate/collaborate care with other disciplines and non-licensed staff as it relates to the outcome and care of the inpatient rehabilitation patient. The Clinical Liaison may assist the PAI Coordinator as necessary to increase efficiency of the unit.
Minimum Position Requirements
Currently licensed in the state of Texas as an RN (BSN preferred) OR Physical Therapist, Occupational Therapist, Speech Therapist, or Therapeutic Recreational Therapist with Bachelor's Degree within specialty.
BCLS certification (AHA)
Pass facility credentialing for Functional Independence Measurement FIM's within 3 attempts upon hire then maintain current certification
Clinical experience in the care and management of the Inpatient Rehabilitation patient.
Must have a minimum of 2 years recent post acute care clinical nursing or therapy experience.
Must have well established leadership skills.
Must possess program development skills & have experience in infrastructure development.
Effective verbal and written communication skills.
Computer and keyboarding skills. (25 wpm minimum)
Ability to work with others to ensure quality patient care.
Strong problem solving skills.
