Job Description

The RN participates as a member of the health care team in delivering quality health care to patients, which supports KP's mission, vision & values. Provides a variety of nursing services both by electronic mail and/or by phone utilizing the nursing process & leadership skills to address acute & chronic needs of Health Plan members & other patients of the KP Health Care Program, as well as in professional interactions w/ employees of the Reg'l Call Center & the NW region. Works under the general direction of the designated supervisor and/or mgr Is responsible for integrating staff education, nursing competencies & professional practice at the department level.
 
Essential Functions:
Provides evidence-based, clinically competent nursing care in a culturally sensitive manner
Plans, implements, manages, coordinates, & evaluates a plan of care across the care continuum
Facilitates member wellness & function & identifies needs for health care interventions
Utilizes appropriate assessment tools, processes, clinical pathways & guidelines to manage patient care
Integrates theory & research-based knowledge in the provision of direct & indirect nursing care
Articulates operational knowledge of procedures, equipment, & skilled nursing tasks as determined by law & the State Board of Nursing
Performs triage & telephone advice independently & w/in scope of the OR and/or WA Nurse Practice Acts
Maintains a safe & therapeutic environment for staff, patients & family members
Participates in research-based projects
Documents the nursing care provided which demonstrates the application of the nursing process & complies w/ dept'l and/or organizational standards
Applies effective oral & written communication skills in interacting w/ patients, families & members of the health care team
Communicates w/ other Health Care Professionals using the Situation-Background-Assessment-Recommendation (SBAR) process
Recognizes the impact of systems on health care delivery
Facilitates coordination of care across all settings
Functions w/in interdisciplinary teams
Applies the knowledge & tools of continuous improvement in clinical practice & the delivery of health care
Utilizes a critical thinking approach to problem solve & make decisions
Applies analytical reasoning, reflection & rational problem- solving skills, using verifiable info, & clinical judgment, in order to choose among alternative solutions to clinical problems
Fosters & uses innovative thinking to create alternative solutions
Demonstrates the ability to analyze, synthesize & utilize data to provide optimal nursing care
Understands & provides competent care to a diverse population
Practices health care team building & leadership skills
Applies leadership skills to facilitate effective team functioning & ops of the dept
Mentors new staff & assists w/ orientation & staff development needs
Collaborates w/ Charge Nurse & Mgr in setting quality standards
Demonstrates effective use of computer-based info & practice mgmt systems for decision support, provision of care, continuous quality improvement & professional development

Participates in ongoing continuing education activities & shares knowledge w/ other health care team members
Demonstrates personal commitment to service & to socially relevant practice
Demonstrates ethical & legal behavior in all professional activities

Minimum two (2) years of experience in acute care or ambulatory care/clinic/call center setting within the last five (5) years
Graduate of accredited school of nursing
Current WA and OR state RN licensure
Demonstrates clinical nursing and leadership skills
Able to type 35 words a minute
Demonstrates effective interpersonal, communication and problem-solving skills
Ability to use telecommunication and computerized information
Demonstrates ability to work within teams and within a dynamic work environment
Customer focused
 
Preferred Qualifications:
Previous experience with population care/case management, triage and advice, preferred
Current or future bachelor's degree in nursing or related field, preferred
