Job Description

The position is located in the Ambulatory Care, Four Corners Regional Health Center, Red Mesa, AZ.  The primary purpose of this position is to serve as Patient Care Coordinator/Case Manager for the outpatient clinical services.  The incumbent reports directly to Director of Nursing. NOTE:  THIS ANNOUNCEMENT INCLUDE PAYMENT OF TRAVEL AND/OR TRANSPORTATION EXPENSES TO NEW NURSES (NEW APPOINTEES) WHO ARE REPORTING TO THEIR FIRST OFFICIAL DUTY STATION IN THE CONTINENTAL UNITED STATES (CONUS).
