Job Description

Responsible for providing independent primary care in the Huntington Ambulatory Care Center to a multi-specialty population of patients. The Nurse Practitioner (NP) coordinates and case manages the patient through the full continuum of care, orchestrating patient flow through a comprehensive care plan, working to ensure that care is appropriate, timely, authorized and conducted in the most appropriate setting. Coordinates the care of the patient serving as the liaison between community physicians and Huntington hospital resources. Provides clinical, professional expertise and leadership for patients, families and staff members. The NP performs history and physical examinations, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests and procedures. The NP functions as an expert clinician, consultant, educator and leader in department specific patient care management, ensuring optimal patient care management and outcomes. Must be a graduate of an accredited nursing program and advance practice MSN, Completion of an accredited program for Nurse Practitioner. Minimum of 3 years experience as a Nurse practitioner in an Ambulatory Care setting. Current California RN license required. Current California Nurse Practitioner license with prescriptive authority. Must have current BLS and ACLS. Must be able to work independently and with a team, and is patient focused. Candidate must have excellent customer service skills with patients, families, physicians and staff and should be able to project a positive self image and be self motivated.
