Job Description

HopeHealth is seeking an experienced Patient Care Manager for its affiliate Hope Hospice & Palliative Care Rhode Island. HopeHealth's headquarters are based in Providence, Rhode Island. Its affiliate members are Hope Hospice & Palliative Care Rhode Island, Hope Palliative Care Center, HopeHealth Massachusetts and Visiting Nurse of HopeHealth.

Under the direction of the SVP, Hospice Operations, the Patient Care Manager of the Inpatient Unit is responsible for the overall care delivered by the Inpatient Unit staff. Has direct responsibility for Nurses, Nursing Assistants, and Administrative Assistant and indirect for team IDG members.

Duties / Responsibilities:

Clinical Services

* Insures that the day-to-day activities and patient care services of the Hulitar interdisciplinary team (IDG) of hospice care professionals are provided in conjunction with other HPC-RI teams and services. Works collaboratively with all other managers to insure timely, competent, interdisciplinary patient centered care for HPC-RI patients and families.
* Assists as a communication liaison between interdisciplinary care providers, patients, referral sources, insurers and physicians, and all others involved in providing patient care. May assist with care coordination. Acts as a clinical resource for all members of the HPC-RI (or Hulitar) Interdisciplinary Team.
* Collaborate with Referral Center to prioritize admissions to the Inpatient Unit. Works closely with liaisons, other staff and HPC-RI Intake manager and staff to provide timely service to referred patients according to agency protocols and patient need.
* Insures availability of adequate resources as well as timely and appropriate assignment of all disciplines to meet the patient care needs. Builds partnerships and resources throughout the organization. Identifies and implements methods to improve efficiency and effectiveness of IDG and other meetings. Responsible for safe effective resource management human, fiscal, and material.
* Functions with demonstrated knowledge and skills necessary to provide care to patients throughout the life span, with consideration of the aging processes, human development stage and cultural patterns in each step of the care process.
* Insures appropriate orientation of Inpatient Unit staff and identifies opportunities for staff to develop new competencies. Insures maintenance of required competencies and credentialing of Inpatient Unit staff.
* Provides direct clinical supervision and evaluation to Inpatient Unit Administrative Assistant's and Nursing Assistants and Inpatient Unit Nursing Assistants, and indirect supervision of Inpatient Unit assigned Bereavement/Volunteer/Spiritual Coordinators, Nursing Assistants, Social Workers, RNs, Pharmacist and Medical Director. Counsels and coaches RN's and other team members (when appropriate) regarding customer focused services and job responsibilities. Provides input to other managers regarding performance on all employees in interdisciplinary team. Assists interdisciplinary team members in resolving problems.
* Pursues professional development opportunities consistent with HPC-RI strategic goals.
* Manages the unit's medication delivery system.
* Acts as Administrator on Call (AOC) for HPC-RI on a rotating basis.

Performance Improvement/Regulatory

* Oversees Inpatient Unit quality/compliance initiatives including chart audit process. Responsible for development of action plans for improvement in identified areas of deficiency.
* Identifies potential risk management concerns and communicates information to appropriate others. Works closely with Quality & Compliance Manager and appropriate others to insure compliance. Establishes and maintains positive relationships with all customers. Handles concerns/complaints effectively and per agency time frames.
* Embraces and uses continuous quality improvement principles and behavior. Participates in and leads quality improvement activities which support the organization's mission, philosophy, customer service and process improvement goals. Develops and monitors key performance indicators as necessary. Implements new programs and makes recommendations for improvement in operational performance. Establishes and maintains an environment of excellence and teamwork. Insures a clean, safe physical environment.
* Insures compliance with regulatory and reimbursement requirements to determine appropriate needs are met and that changes in treatments and physician orders are implemented.

Program Development

* Collaborates with Operations Team in formulating, revising, implementing and evaluating organizational policies, procedures, goals, and objectives.
* Responsible for Inpatient Unit budget development and monitoring and adheres to budget guidelines. Works closely with finance and supervisor to insure oversight of fiscal responsibilities in Inpatient Unit.
* Assists in orienting new employees to HPC-RI. Identifies ways to improve quality field placement experience for all approved HPC-RI health care visitors and students.
* Supports HPC-RI Foundation fund development activities, especially as they relate to Inpatient Unit.
* Assists in the development of educational programs for staff including staff from other teams.
* Collaborates with clinical and non-clinical managers, staff, and departments in developing and implementing innovative programs to better meet patient, family needs, and community needs to achieve organizational strategic goals.
* Supports HPC-RI Foundation Fund development activities, especially as they relate to Inpatient Unit.
* Consistently apply the HPC-RI values in all aspects of work, complying with established HPC-RI mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance.

Education / Experience

* Registered Nurse with Bachelor's Degree in Nursing.
* Currently licensed as an RN through the State Board of Nursing in Rhode Island required. Additional Massachusetts RN license preferred
* Evidence of applied knowledge in all state and federal insurance standards, regulations and accreditation standards.
* Five (5) years clinical experience with in-depth clinical knowledge to include a minimum of two years nursing experience in a Home Care/Hospice setting and progressive management experience/success
* Excellent interpersonal skills with proven ability to work effectively with others in problem solving.
* Demonstrated leadership ability
* Effective organizational skills
* Must be able to proficiently use a computer. Knowledge and working ability with Microsoft Word and Excel in a Windows environment.
