Job Description

In close collaboration with the Executive Medical Director of the Hoag Neurosciences Institute, it’s Neurosurgeons, other Nurse Practitioners, and other members of the healthcare team, the Neuroscience Nurse Practitioner is responsible for assisting in the expedient  multidisciplinary care of patients for Hoag Hospital and its affiliates (the “Organization”), coordinating seamless patient care from admission through discharge.  Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures (drain and suture removal) and facilitates the discharge process.

 

The Nurse works in collaboration with the entire patient care team providing multidisciplinary care of patients adding clinical, professional expertise and leadership for patients, families and staff members.

The Nurse Practitioner also functions as a knowledgeable clinician, consultant, educator and leader in Neurosurgical patient care management, ensuring optimal patient experience and outcomes. In this role, the Nurse Practitioner maintains and continually improves systems for coordinating and communicating the bundle of care throughout the entire episode of hospitalization.

 

The Nurse Practitioner displays and practices a work-style reflecting the mission, vision and values of the Neuroscience Institute, as well as the Organization. Participates in the continuous quality improvement process. Maintains a working knowledge of departmental standard operating procedures, including the use of specialized instrumentation, Quality Control requirements, and preventive maintenance.

 

Essential Functions:

  • Works knowledgably and collaboratively in the Organization to optimize the multidisciplinary care of patients.
  • Provides clinical, professional expertise and leadership for patients, families and staff members.
  • Functions as a knowledge expert in neurosurgical care, consultant, educator and affiliate staff/nursing leader in Neurosurgical patient care management, ensuring optimal patient experience and outcomes.
  • Maintains a system for coordinating and communicating the entire episode of care throughout the hospitalization.
  • Acts as a consultant in clinical setting, maintaining and promoting standards of care.
  • Collaborates with all members of the interdisciplinary team to achieve seamless movement of the patient between the emergency department, nursing floors, operating room, and critical care components of the organization.  
  • Assists with education of nursing and other ancillary staff.
  • Maintains active involvement in nursing and hospital committees as necessary. 
  • Coordination of care includes:
    • Attendance at daily nursing huddles to review patient caseload.
    • Assisting with communication between the nursing team and the physicians and their care extenders.
    • Facilitating access to services and timely movement through the hospitalization period.
    • Transmission of patient information to healthcare team members to enable them to quickly customize care.
    • Daily rounding on Neurosurgical patients in the hospital.
    • Writing orders for medications and diagnostic tests under the oversight of the Neurosurgeons.
    • Performing minor procedures (drain maintenance and removal, suture removal and wound care) under the oversight of the Neurosurgeons.  
    • Monitors and actively initiates interventions to meet the expected LOS targets for identified patient population.

  • Arrange admission and transfers of patients*
  • Performs history and physicals*
  • Dictates history and physicals*
  • Daily round on assigned patients and writes progress notes with plan of care*
  • Dictates discharge summary*
  • Furnishes medications (non-narcotic) and durable medical equipment (DME)*
  • Furnishes controlled substances III, IV, V with appropriate licensure/certification*
  • Furnishes controlled substances II with appropriate licensure/certification*
  • Orders blood products*
  • Orders laboratory tests*
  • Orders radiological, ultrasound studies, nuclear medicine studies, neurological studies*
  • Performs drain and suture removal*
  • Orders and management of oxygen therapy, orders respiratory treatments, orders pulmonary tests*
  • Orders and may administer medications and IV therapy with the ability to assess and adjust for the appropriate dosing and effects of the medication.
  • Documents in the patient record utilizing documentation guidelines, standards of care and patient goals.
  • Acts as a consultant in clinical settings, maintaining and promoting standards of care
  • Manages and coordinates caseload of care throughout the entire patient hospitalization and illness spanning each clinical area.
  • Assists nursing in coordinating care for assigned patient population.  Coordination of care includes:
    • Pro-active outreach to establish contact with newly diagnosed patients
    • Assisting patients and family with navigation inside and outside the Hoag system
    • Facilitating access to services and timely movement through points of care
    • Transmission of patient information to healthcare team members to enable them to quickly customize care at each point of care in the delivery process
  • Performs follow-up at specific intervals in the outpatient setting and at points of care transition

  • Applies positive verbal and non-verbal skills to achieve service goals of customer satisfaction and positive outcomes.
  • Demonstrates an ability to organize and incorporate new ideas, routines, procedures and skills.
  • Coordinates with other departments and services inside and outside the hospital to achieve the patient’s individual health goals and neurosurgical outcome standards.
  • Performs any other duties per approved Nurse Practitioner Standardized Procedure.

 

 

Essential Functions

  • Coordinates care for complex neurosurgical patients incorporating age specific assessment date in the development of individualized plan of care.
  • Uses specialized equipment for neurosurgical patients.
  • Demonstrates clinical competence in identification, assessment and intervention for neurosurgical patients.
  • Collects and reports relevant clinical data.
  • Perform other duties as assigned.

 

  • Clinical Care:  Ensure patient care is delivered and the unit is maintained in accordance with professional regulatory standards. Demonstrate the knowledge and skills necessary to provide care appropriate to the population as outlined in the job description and competencies
  • Performs other duties as assigned.

 

Education, Training and Experience

 

Required:

  • Master’s degree in Nursing.
  • Current license in good standing to practice as a Registered Nurse/Nurse Practitioner in California along with Continuing Education Requirements.
  • Furnishing license issued by the State of California and personal DEA number.
  • Minimum two (2) years recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment.   Experience in Neuroscience considered in place of Nurse Practitioner experience.
  • BLS and ACLS certification

Preferred:

  • Two years recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment, related to the care of Neurological or Neurosurgical patients.
  • National board certification as a Nurse Practitioner in area of specialty.

 

 

Skills or Other Qualifications

 

Required:

  • Advanced clinical knowledge and skills in the care and management of Neurosurgical patients.
  • Evidence of satisfactory performance and completion of Medical Staff proctoring requirements within 6 months of hire.
  • Availability and appropriate use of consulting physician on medical staff for the Organization.
  • Performs according to approved standardized procedures and/or protocols.
  • Assessment of patient’s status with appropriate findings and initiation of appropriate tests, medications and therapeutic interventions.
  • Excellent communication skills; customer focused; excellent attention to detail; able to prioritize tasks and meet deadlines; strong interpersonal skills; able to work in a team environment.
  • Proven excellent customer service skills, including the ability to progressively probe, analyze, identify root causes of problems, provide practical solutions, and negotiate resolutions.
  • Solid computer technical skills required, including proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint, and Visio).

  • Qualified bilingual or multilingual candidates.
