Job Description

The Orthopedic Navigator is a registered nurse responsible for the coordination of patient care from pre-admission thru discharge.  Coordinates the pre-admission process to ensure the patient assessment is completed according to hospital standards; notifies the physician with abnormal clinical findings; educates patient and family/significant other on the hospital experience including preadmission and post discharge planning; implements, coordinates, and monitors and evaluate care options, services and resources. Proactive in finding ways to enhance the patient experience clinically and in service in order to provide an exceptional patient experience.


  • Collaborates with members of the orthopedic team to provide seamless care to the patient from initial surgical schedule through discharge.
  • Responsible for the organization and management of the patient through the care event including follow-up for 2 years post procedure, collecting functional and service information
  • Develops and maintains key relationships with the medical groups, physicians, staff and the community through educational events, organization sponsored events.
  • Educates and mentors staff, physicians, the community as well as patients and their caregivers regarding services.
  • Collaborates with ancillary departments to facilitate access to community based resources such as extended care facilities, rehabilitation services.
  • Develops captures and analyzes outcomes to evaluate effectiveness of and improve patient care and satisfaction. Assists and practices in compliance accreditation bodies (i.e. clinical competencies, policy and procedures, federal and local regulatory bodies).
  • Clinical Care:  Supports team providing patient care. Ensures the unit is maintained in accordance with professional regulatory standards. Demonstrates the knowledge and skills necessary to provide care appropriate population as outlined in the job description and competencies. Able to support and assist with daily patient care as needed.  
  • Performs other duties as assigned.


Education, Training and Experience

Required:        Minimum 2 years acute care experience.

Preferred:       5 years nursing experience. 3 years experience in area of specialty. BSN preferred. Prior experience as a case manager or heath educator.



Skills or Other Qualifications

Required:        Strong written and verbal communication skills. Must have critical thinking skills, be able to problem solve independently, work well in a team environment. Demonstrates effective communication, interpersonal skills, organizational ability, time management, and knowledge of proper body mechanics, safety measures and infection control, ability to follow instructions, compliance with regulatory guidelines and rules, positive, can-do, pro-active attitude.  Current orthopedic experience in the surgical out-patient arena or hospital based care. Familiar with community resources and hospital processes, structure and function. Strong knowledge of orthopedics, surgery, out-patient resources and coordination of services, musculo-skeletal system and physiology. Detail oriented, strong documentation skills, clinically astute. Comfortable with technology and innovation. Ability to carry clinical and technical skills across various clinical situations, not necessarily just orthopedics/surgical. Strong knowledge of pathophysiology. Understands and is able to coordinate a variety of care-providers and schedules to maximize the patient and family experience.

Preferred:       Competent in Microsoft Office, keyboard a minimum of 45 WPM. Current use or recent past use of an electronic charting method/EMR.


License and Certifications

Required:        Current California license as a registered nurse, in good standing. Current BLS prior to employment.

Preferred:       Active NAON member and certified (ONCB) or Medical-surgical certification. ACLS preferred.
