Provides a wide range of clinical social work services to the Urgent Care, Primary and Specialty care clinics Wednesdays through Saturdays at the Bellevue Medical center with primarily urgent care work on Saturdays.
Clinical Social Workers at Group Health have a key role in supporting medical teams in delivering evidenced-based wellness and lifestyle management interventions. Evaluates psychosocial assets and deficits for the purpose of enabling the patient to maintain the greatest level of independence while improving health outcomes. Coordinates with others to establish a patient-centered plan of care. Provides assessment, brief risk intervention and short-term counseling, educates and assists in navigating resources for care giving needs.
The ideal candidate will bring a strong behavioral and socioecological lens to patient care, and have demonstrated success as part of a highly adaptable, integrated healthcare team.

Hours are 10:00 am - 8:00 pm - Wednesday through Saturday.
Minimum Education Requirements
Masters Degree Masters of Social Work (MSW).
Minimum Knowledge/Skills Requirements
Intermediate Excellent written and verbal communication skills. Demonstrated success as part of a multi-disciplinary team and commitment to diversity.
Intermediate Knowledge of social case management, conflict resolution and team building practices. Familiarity with Microsoft Office. Familiarity with electronic charting.
Minimum Experience Requirements
1 year previous social work experience
4 years Post-Masters experience. Care coordination experience in managed care system, in-/outpatient setting, skilled nursing or long-term care facility.
Minimum License/Certifications Requirements
At time of hire Licensed Independent Clinical Social Worker (LICSW) for MSW in Delivery System Operations.
