Job Summary

The AccessHealth (AH) Care Transition Coordinator (CTC) identifies a targeted low-income, uinsured patient population in both hospital and community settings for enrollment in AH.  The CTC provides healthcare navigation services to enrolled AH patients, coordinates medical home placements and first appointments, collaborates with a multi-disciplinary team to address the social determinants of health, and arranges supportive services and referrals to community partners for improved health outcomes and to prevent avoidable hospital admissions/emergency department visits.  The CTC may provide limited health coaching and advocacy to improve AH patients' appropriate healthcare utilization, and may assist them with applications for Medicaid, prescription assistance, and /or other benefit programs.  The CTC will be expected to minimally make biannual contact with patients on an assigned caseload, and to participate in community outreach events, assist the AH Program Director with reporting to The Duke Endowment, the University of South Carolina, and state agencies, and to perform other administrative duties as assigned and to meet the goals of the program.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Education

Bachelor's Degree

Minimum Experience

2 years- Experience in a Care Coordination, Case Management,or nonprofit and/or community experience.

Other Required Experience

 Ability to master Access Health South Carolina's software programs including the data tracking software Phases in conducting daily tasks with proficient skills in a window based environment. - Preferred

Knowledge of healthcare systems and operations as well as the federal insurance exchange and Benefits Bank.- Preferred

Knowledge of local healthcare options and resources including mental health.  - Preferred
