Good Shepherd Penn Partners (GSPP) is the "Official Therapy Provider for Penn Medicine." GSPP is a joint venture between Good Shepherd Rehabilitation Network and Penn Medicine. The Penn Medicine Hospitals include: Pennsylvania Hospital (PAH), Penn Presbyterian Medical Center (PPMC) and the Hospital of the University of Pennsylvania (HUP).

Therapy services (OT, PT, SLP) are provided to a variety of patient populations including Orthopedic, Trauma, Heart and Vascular, Neurological, General Medicine and Surgery, solid organ and blood transplantation and patients with medical complexity. The Penn Medicine system has the highest Case Mix Index (CMI) in the tri-state area, therefore early interventions for those with medical complexity is the standard of care. The hospital network has specialty recognition including: Nursing Magnet status, TJC stroke accreditation at Tier I and II, TJC VAD certification for destination and bridge to transplantation, an ACE unit, one of the largest Lung transplantation and bone marrow transplantation centers, and more. Each of the hospitals maintains a unique identity that is rich in history and yet strives to provide excellence and little variation in care across the health system. Additionally, PAH and PPMC offer post acute rehabilitation services on their campus. The GSPP therapists coordinate discharge for the patients post acute care and have the entire rehabilitation continuum of care available within the system including skilled, sub-acute, LTACH, acute rehabilitation, home care and outpatient therapy. Consultation and collaboration with a multidisciplinary team is evident in all the hospital settings. As GSPP therapists, we partner with each other, Penn Medicine as well as other health care practitioners to provide evidenced- based care focusing on clinical best practice and service excellence. Our therapists participate in education of members of the healthcare team, partner to contribute to research related to rehabilitation and maintain and provide quality care focused on outcomes.

Physical Therapist Responsibilities:

* Provide Physical Therapy services in the hospital-based setting, inclusive of performing examination and evaluation, determining a diagnosis, prognosis and developing individualized plan of care with appropriate recommendations for discharge.
* Determine functional diagnosis by utilizing physician referral, medical records and physical therapy examination.
* Establish goals to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients.
* Include patient and family members in program planning and provide interventions utilizing a patient focused care approach
* Carry out interventions with patient consent.
* Complete documentation elements that adhere to rules for professional practice act and guidelines for regulatory and reimbursement agencies. Clinical documentation is performed via electronic documentation system.
* Perform reassessments at regular intervals and communicate patient progress to referral sources.
* Discharge patient from physical therapy when goals or projected outcomes have been attained.
* Provide for appropriate follow-up care and discharge communication to referral sources.

In addition to clinical care responsibilities, therapists participate in professional activities including: supervise, train and assess therapy students during onsite clinical internships/affiliations; keep abreast of developments in the field by reading current literature, collaborate with colleagues, and participate in professional conferences; and provide education to staff. Staff participate in many interdisciplinary committees within each hospital helping to promote a culture of mobility and function. Additionally, therapists participate in health promotion programs and community based events to promote patient/client health, and prevent physical disability.
