Job Description

Frontier Behavioral Health (FBH) is a large, well respected community mental health center located in eastern Washington. We are seeking experienced professionals to work within a team to provide integrated mental health services.

Clinician II & III Positions

Master's degree in psychology, counseling, social work in the state of Washington; 2 years experience in provision of behavioral health services. Must be eligible for Washington State Registration (Clinician III applicants must be licensed in State of Washington); must meet criteria for Mental Health Professional. Positions provide counseling and care coordination services to clients in various programs (Adult Recovery, WISe, Adult Outpatient, Youth & Adult Services - Howard).

Care Coordinator - Health Homes

Required - Master's degree in social work or the behavioral sciences with 1 year experience working in a behavioral healthcare setting, or Bachelor's degree in the behavioral sciences with 5 years of related experience working in behavioral health care or a health care setting. Some experience working with children is helpful.

Provide Care Coordination to individuals who have multiple chronic conditions, have been identified as medically vulnerable and are at risk for ongoing high usage of emergency care. Responsible for engaging with assigned members, assessing patient needs, providing education, support, coordination of care and referrals to community resources, with the goal of helping members attain or improve self-management of their conditions. Responsible for creating and reinforcing Health Action Plans to address social and chronic medical and behavioral conditions. Also responsible for coordinating referrals to community services and clinically indicated services such as social services, mental health care, substance abuse treatment and long term supports and services. Most work is off-site, typically in members' homes.
