Job Description

The Senior Services Home Care RN functions as part of an interdisciplinary team to provide excellent patient care. Plans, organizes, coordinates, and provides skilled nursing care to home care patients in assisted living and memory care communities according to a plan of care, including advanced nursing interventions related to the patient's needs.

New hires may select between a 24 - 40 hr/week schedule.

Scheduled days are Monday-Friday, 8a.m. - 5 p.m. plus rotating weekends/holidays. Job Qualifications: Qualifications:

Must have experience working with geriatric patients, be knowledgeable regarding treatment of dementia symptoms and behavioral/cognitive changes associated with aging.

Minimum Qualifications:

Associates's degree in Nursing - Will accept Associate's degree RNs with at least 6 months previous RN work experience

Licensed as an RN and eligible to practice in MN

Current CPR certification (BLS for Health Care Providers)

Will accept new grad RNs with completed BSN degrees AND Public Health Nurse (PHN) Certification


BSN or current active pursuit of BSN

Public Health Nurse (PHN) certification
