Hours: 8am-4:30pm

Job Summary:

Reporting directly to the Office Manager and Clinical Support Specialist this position will be responsible for assisting the Emerson PHO staff in a wide range of important duties related to the day-to-day operations of the PHO. This person will aid the organization in a variety of areas, including, but not limited to: Care Management, Quality, NCQA Patient Centered Medical Home (PCMH) Recognition, Meaningful Use and PHO Office administrative functions.

Job Functions:

* Work with the Emerson PHO Project Manager and related team members in conducting patient chart reviews of primary care practices for PCMH recognition.
* Assist the PHO Project Manager with other tasks related to PCMH recognition, including, but not limited to, organizing and mailing letters to identified patient populations.
* Help the PHO office organize paper documentation related to its provider staff by scanning and storing pertinent documentation in an electronic format.
* Assist in the organization and entry of data related to PHO provider staff into a defined database.
* Provide support to the Care Management team by assisting with administrative tasks, such as the sending of new patient letters and data entry of home care authorizations.
* Assist the Provider Support Specialist in data mining tasks to facilitate documentation related to patient quality measures.
* Maintains and supports patient's rights to care, privacy and respect.
* Other duties as assigned from time to time.

Minimum Qualifications:

Education: High School diploma required. Pursuit of a college degree in healthcare or related field preferred.

Experience: Previous experience not required.

Licensure and/or Certification: N/A

Skills:

* Excellent customer service skills.
* Must be able to read, write, and communicate in English.
* Computer skills required.
* Ability to maintain confidential information.
