About Elderberry Healthcare and Physical Therapy

Elderberry Healthcare and Physical Therapy (EHPT)  is an outpatient practice that provides therapeutic services to our clients in their home.  We focus on assisting adults with maintaining their physical mobility, mental stamina, and activities of daily living. Our clinicians work hard to help our clients reduce their risk of falls and maintain their independence both in the home and in the community.  We are looking for highly motivated therapists to join our team in the greater Los Angeles area. 

Elderberry Healthcare PT is seeking an Occupational Therapist to treat patients in the outpatient setting that brings services into our clients homes. Elderberry Healthcare PT is Therapist owned and operated so we understand what it takes to be in the field. We thrive on providing exceptional services and evidence based patient-centered care to clients throughout Southern California.

We strive to provide our new employees with a structured on-boarding process to help you become assimilated quickly, and make your daily work easier and more efficient. Since we are considered outpatient care, documentation requirements for Medicare is made simple using our EMR system.

Clinicians choose their own schedule and coverage areas; streamline electronic payments once a month. Use electronic medical records (WebPT); We simplify your documentation process and notify you via text on everything from the administrative side so that you can focus on patient care.

Occupational Therapist (OT) Job Description/Requirements:

  • Evaluation/Treatment of Patients: Complete evaluation and treatment of patients in compliance with agency policy, law and regulation and within the Occupational Professional Scope of Practice. Assist in evaluating level of function and in developing the plan of care.

  • Evaluate the home environment and make appropriate recommendations. Help patients and families modify tasks, and use equipment to improve participation with their ADLs and improve their quality of life.

  • Provides Continuity of Care: Ensure appropriate planning for patient discharge/reassessment and referral to other services according to agency policy and in compliance with law and regulation. Advise, consult and, when appropriate, instruct family and other agency personnel in patient’s therapy program

  • Communicates: Communicate with care coordinator and other health care professionals caring for the patient to ensure uninterrupted patient care. Observe records and report information on the patient’s condition to the attending physician and in the patient’s health record.

  • Supervise COTA  when assigned

  • This is an independent contractor position and the pay is $60-$75 per visit. Rate is based on 5 Medicare units billed. 

  • Monthly gas card, incentive based raises, monthly raffles (food cards, clothing cards, swag)

  • EMR system used: WebPT

  • No experience required 

  • New Grads welcome

  • Locations: 

    • Los Angeles area ( Hollywood, West Hollywood, Korea Town, West LA, Santa Monica, Culver City, Beverly Hills, DTLA, Inglewood, Pasadena, Burbank, Long Beach, Glendale, Woodland Hills, The Valley, Alhambra, Covina Pomona)

    • Orange county (Anaheim, Orange, Irvine, Newport, Fullerton, Garden Grove, Costa Mesa, Buena Park Cerritos, Huntington, Westminster)

  • Currently we have a high volume of patients in:Palos Verdes, ready for new OT! 

Minimum Required Qualifications:

  • Current CA OT License

  • Valid California Driver License/ with transportation insurance

  • EIN/ with Business License required or Social Security Number

  • Mal-Practice liability Insurance

  • Physical within 2 years, TB test within 1 year

  • CPR certificate 

  • Car Insurance 

  • COVID-19 Vaccination documentation
