Geriatric Support Services Coordinator (GSSC) Full-Time: This Elder Services employee works collaboratively with the Primary Care Team from Fallon Health Navicare SCO Program.

The SCO GSSC (Geriatric Support Service Coordinator) plays an integral part of the case management functioning of the agency. The SCO GSSC will provide initial assessment and ongoing assessments of the SCO client's needs including eligibility for any relevant Elder Services of Berkshire County, Inc. (ESBCI) services and other community resources. Furthermore, the SCO GSSC will develop a service plan based on their assessment of member needs, with the goal of addressing areas of concern highlighted through the assessment.

This position requires the ability to work autonomously, multitask, conduct home visits and family/provider meetings, telephone contact and regular documentation. The SCO GSSC is responsible for working collaboratively within ESBCI as well as externally with client formal and informal supports, SCO representatives, vendors, potential referral sources and other community agencies. SCO GSSCs actively participate in ESBCI's organizational life.

Required: at least a bachelor degree and two years professional experience in the care of persons over the age of 65, with at least one year involving work in a setting where persons over 65 receive health-care services (such as a hospital, nursing facility, a community health agency, or interdisciplinary program within an ASAP); and have the knowledge and skills necessary to work successfully in the interdisciplinary team and to provide or arrange for community long term care services and supports.

Job Developer (Part-time):

Highly organized, detail-oriented individual to promote the Senior Community Service Employment Program (SCSEP). The successful candidate will have experience in job development or related field. Recruit, interview and enroll eligible candidates to the program. Conduct outreach activities throughout the county. Work closely with area businesses to promote the SCSEP program and permanent employment for the participants. This individual must possess strong communication and excellent computer skills and have the ability to function effectively within a team. Bachelor Degree Preferred. Must have reliable transportation. Travel throughout Massachusetts is required.
